America First Global Health: an African reality check
๐This is not global solidarity. Itโs U.S. security policy wearing a health coat.
๐Aid now follows American risk, not African need.
๐Bilateral deals mean leverage: comply, or funding shrinks.
๐Commodities are funded; doctors are burned out and leaving.
๐Data flows upward. Responsibility stays local.
๐TB, HIV, malaria are protected... mothers, newborns, NCDs are expendable.

That means out of 100 trained health professionals:

So nearly 1 in 3 health professionals is not serving patients.
This isnโt just a licensing issue. Itโs a retention, motivation, and system failure problem.

That means:

~67,700 are licensed but not providing care

What this means in plain terms

Lawyers Perspective!
When several respected young professionals die in a short time in the same workplace/ community, it naturally triggers questions like: Is there a common underlying cause (illness? stress? workplace conditions?)
Could these deaths be due to exposure, accidents, or even intentional harm?
Why Gondar and not another hospital?
These questions have to be investigated.
My condolences for fellow doctors, families and friends.

Nowadays, medical schools are opening everywhere, everywhere except แฐแแ แซ. Some of these schools graduate medical students without basic facilities like cadavers or anatomage tables (digital 3D anatomy simulation table used for teaching human anatomy). Yet their graduates often come out with inflated grades and end up competing for positions against graduates from older, more rigorous medical schools.
Ironically, students who were trained under tougher conditions get disadvantaged. Academic recruitment now demands a CGPA of 3.25 for males and over 3.0 for females and >=B grade in all internship attachements, making it difficult for strong graduates from traditional schools that use stricter grading systems and older academic cultures to compete for lecturer positions with those from แฐแแ แซ med school.
In some universities, the patient load is overwhelming while staffing remains minimal, UoG is a clear example. It serves as the only major referral hospital for the entire Gondar area. The workload, stress, and burnout are real. Support systems are weak and, at times, openly hostile to juniors.
The way the university senate and militia responded during our movement for fair pay and safe working conditions shows only a small part of the deeper problem.
Public perception in ๐ช๐น doesnโt help either. Many people admire herbalists like โแแจ แฝแแซแโ more than highly trained Ethiopian doctors, and showy physicians more than those who quietly work hard (the way they were trained in medical school).
On the other side, repeating the same mistake like opening แฐแแ แซ undergrad med school...many seniors/lecturers are opening residency programs in newly established universities where they are deployed. This often comes at the cost of quality, repeating the same mistakes seen in undergraduate education. Many newly established postgraduate programs canโt even pay residentsโ detachment allowances.
These new residency programs are opened not to deliver quality training or service, but to free up time for private practice while residents handle the routine workload. Quality is rarely a priority. Residents who question this eventually face consequences, so most stay silent and the cycle continues.
This is exactly how our once-strong undergraduate medical schools were compromised and now the same is happening in postgraduate training. It reflects how medical education is expanding while quality steadily declines.
Everything is messy. We can only hope for a genuine solution.
To students from old, reputable universities: stay strong and be proud of where and how you trained.
To lecturers in those institutions: build strong, supportive systems for your students. Make sure they are recognized and prioritized after graduation for enduring such demanding training unlike those graduating from places like แฐแแ แซ medical school.

Workload Classification
Low workload (โค40 hours/week)
๐Typical of outpatient-focused roles or light clinical rotations
๐Allows adequate rest and recovery
Moderate workload (41โ60 hours/week)
๐Common in standard inpatient services
๐Acceptable when duties are well structured and supervision is adequate
High workload (61โ80 hours/week)
๐Characterized by heavy clinical service
๐Associated with increased fatigue and reduced learning efficiency
๐Acceptable only for short periods.
Very high / excessive workload (>80 hours/week)
๐Strongly associated with burnout and increased risk of medical errors
๐Not recommended, especially for interns and residents
In most medical schools, interns work five regular weekdays and undertake two to three duty shifts per week, amounting to approximately 72โ96 working hours weekly. This places interns within the high to very high workload categories.
