حالة اليوم:
RCT For Lower 2nd Molar..

بعد ازالة العصب والانتقال الى أخذ الاطوال بجهاز الApex-Locater
كان الجهاز بيعطي نفس القراءة لجميع القنوات (19mm)

بعدها انتقلت لخطوة التأكد من الاطوال بالاشعة
واثناء ادخال الفايلات في ال Mesial Canals
لاحظت انه لما ادخل الفايلات مع بعض بالقنوات
كان الفايل الاول يدخل على نفس الطول(19mm) بينما الفايل الثاني كان يدخل على طول 18mm اي ناقص 1mm

ولما ارجع ادخل الفايلات بالتناوب كانوا يوصلوا بالقناتين على نفس الطول 19mm

وهذا بيعني ان Mesial Canals
بتنتهي ب Apical Foramen واحدة في أخر 1mm

في مثل هذه الحالات اذا حضرت القناتين وعملت Shaping الى Full Working Length (19mm) ب اكون عملت Over-Preparation للApical Part،
وبالتالي بيعملنا مشاكل مثل ال Apical Transportation

الحل:
نعمل Shaping لواحدة من القنوات الى
Full Working Length (19mm)
بينما القناة الثانية نعمل Shaping الى منطقة الالتقاء بين القناتين اي الى طول 18mm

وهكذا بنتجنب حدوث ال Over-Preparation لل Apical Part.

ملاحظة:
قناة ال Distal في الapex locater
كانت تظهر على طول 19mm
وعند عمل الاشعة للتأكد ظهرت وكأنها ناقصه 1mm،
وفي هذه الحالات بنتبع جهاز ال apex locater لان غالبا ال apical foramen بتكون Laterally

- اول و اهم حاجه في اي procedure بتعملها سواء adult او pedo و اللي هيحددلك شغلك هو ال diagnosis بتاعك .. في ال pedo ال diagnosis بتاعك ممكن يحدد اذا كنت داخل علي pulpotomy و لا pulpectomy ..

- لو الالم اللي بيجي للطفل الم مؤقت .. بيجي بس وقت الاكل و بيروح علي طول .. بيجي مع المياه الساقعه او مع ال sweets و بيروح علي طول .. ف احنا كدا في مرحلة ال reversible pulpitis .. ف انت كدا بتحط احتمالية ال pulpotomy

- لو الالم مستمر و هنا بيبقي في indication لان حاصل wide spread لل inflamation في ال pulp وواصل لل radicular pulp tissue و هنا بنبقي وصلنا لمرحلة ال irreversible pulpitis و بنحط احتمالية ال partial pulpectomy او pulpectomy ..

- لو الالم Spontaneous و دا بيبقي throbbing and constant pain و بيحصل خصوصا بليل و دا بيبقي indicated ل progressive or extensive degeneration of the pulp و هنا بنبقي متاكدين اننا داخلين علي pulpectomy

- وانت بتعمل clinical examination :
١- لو في pain with percussion و هنا ال periodontal ligaments بتبقي inflammed و هنا ممكن تحط صابعك علي السنة و تضغط و تشوف ال response بتاع الطفل و هنا بنروح برضو لل pulpectomy .. الا لو ال permanent عامله resorption في ال primary tooth او عمر الطفل بيقولك ان دا وقت ال eruption بتاع ال permanent ف انت هنا هتخلع ال primary

٢- لو لقيت في swelling related للسنه سواء intraoral او extraoral او لقيت fistula ف السنه في الحاله دي بتكون non vital .. بت confirm ال diagnosia بتاعك ب ال x ray عشان تشوف هل حاصل root resorption و لا لا .. لو مفيش بتروح لل pulpectomy

٣ - لو لقيت mobility في السنه .. ف انت قدام احتمالين .. يا السنه قربت تتخلع عشان ال permanent هتطلع .. يا في lesion تحت السنه ..

- طبعا و حتما ولابد لازم تاخد pre-operative x ray عشان ت confirm ال diagnosis بتاعك .. لان ممكن متحتاجش لكل دا و تكون السنه محتاجه تتخلع .

- طيب انت كدا عرفت قبل ما تخش تشتغل انت داخل علي pulpotomy و لا pulpectomy .. طب لو لسه مش عارف و كل اللي عارفه انك داخل تعمل pulp therapy و خلاص .. تعمل ايه ؟

بتبدا تخش في ال tooth ب large round bur عشان تحس بال drop و عشان تتجنب ال perforation في ال pulpal floor .. لان ال large bur هياكل في area مش ف point زي لو استخدمت small round bur ...

عملت ال drop بتاعك ف هتبدا تعمل derofing .. ممكن تستخدم هنا ال endo Z bur و ممکن تكمل بال large round bur عادي .. و لكن لازم تعمل complete derofing .. عشان ت locate ال canals ...

- عملت ال complete derofing .. هتبدأ تشيل ال coronal pulp tissue .. ب large round bur او sharp excavator .. و لكن يفضل ال sharp excavator عشان تتجنب ال perforatoin ..

- بتشيل كل ال coronal inflammed pulp tissue و خصوصا اللي فوق ال canals عشان تشيل كل ال inflammed pulp tissue ..
