كلام مهم جدا هتلاقيه فى صفحة رقم ٢٤٣ من كتاب Easy endodontics 👇
على الرغم ان بعض الدكاترة مش بيهتموا اوى بموضوع ال Coronal flaring الا انه يعتبر من اهم الخطوات فى ال root canal preparation 🤚
معنى ال Coronal flaring اننا نوسع ال coronal third من ال canal قبل ما نعمل اى حاجه
قبل حتى ما ناخد working length 😶👍
طب هيفيدنا بايه ال coronal flaring 🤔👇
✅ اول حاجه هيخليك تقدر توصل لل apical third وال apical foramen بسهولة وبالذات لو فيه apical curvature
✅ بردو هيقلل ال stress على ال files سواء manual or rotary وبالتالى هيقلل فرصة حدوث ال preparation errors واللى اهمهم طبعا ال file separation وال ladge
✅ كمان لما توسع ال coronal third هتسمح لكمية كبيرة من ال irrigant توصل لل apical & middle areas وبالتالى هيحصل disinfection كويس
✅ اخر حاجه ومهم بردو انك لما تبدا بال coronal flaring انت كده بتخلى كل ال infected pulp & debris انها تطلع coronally بسهوله بدل ما تتراكم وتنزل ناحية ال apex 🤝

ممنوع تلمس ال floor بتاع ال access cavity بال end cutting bur لانك لو عملت كده طبعا هتزود فرصة انك تعمل pulpal floor perforation لكن بردو حاجه مهمة وهى انك لو لمست ال pulpal floor ب bur فانت ضيعت على نفسك حاجة مهمة جدا وهي ان ال natural floor بتاع ال cavity بيبقي convex فلما تيجي تدخل file في ال canal orifice هيخش معاك بكل سهوله ومن غير تعب ✌👌
لكن لو لمست ال floor بال bur كده هتحول ال convex floor الي flat وده هيخلي صعوبه شديدة لما تيجي تدخل الفايل في القنوات 🧐

السلام عليكم ورحمة الله وبركاته 🌸

موضوعي اليوم على "ferrule effect "🧐

عندي سن badly decay وقبل مانخدملها عصب منشوف هل السن هادي restorable أو لا ؟ من الحاجات اللي منحطها في بالي ان هل نقدر نلبسها بعد الRCT أو لا ؟

ومن هني نبدو قصتنا على الferrule effect

شن معنى ferrule ومنين جتنا الكلمة ؟

هي كلمة لاتينية بمعني اسوارة "حُلقة" معدنية أو حديدية😂
Ferrum =iron
Viriola =bracelet
شبهو الأسوارة هادي بالكراون لما يلف على السن

كيف هكي🤔 شن دخل الكراون مش ferrule زي

مانعرفو انه هي
"Remaining tooth structure above gingival margin"

قالو لا ديما المصطلح هذا ينفهم خطأ
"Misinterpretated"
وتعريفها هو :-
The actual bracing of the complete crown over the tooth structure constitute the ferrule effect .in other words➡️protection of the remaining tooth structure against fracture.
Or
" a 360° of the metal (Or porcelain)collar of the crown surrounding the parallel wall of dentin (remaining tooth structure) extending coronally to shoulder of the finish line.

اذا هو عبارة عن كراون يلف على المتبقي من السن بشكل كامل باش يقوي ويحمي السن من الكسر .
طبعاً منحكوش عالكراون كامل فقط الجزء اللي لاف على السن 🤚

وقاعدة ferrule effect تقول ⬇️
Embracement of the tooth by crown restoration.
معناها ferrule effect منقصدوش بيها اللي قاعد من السن فوق اللثة أو finish line ؟

لا لا اللي قاعد من السن هذا شرط باش يتحقق ferrule effect 》》مش هو ferrule في ذات نفسه 》》ولأن effect ميتحققش الا بيه ف ماشية فيه الكلمة 😁

و نقولو على الجزء المتبقي من السن هو
"Ferrule tooth structure not ferrule effect"

》》باهي ليش مهمة عندي ferrule ؟

#:حاجة مهمة جدا تحددلي نجاح أو فشل artificial
crown

لان لو معنديش tooth structure مش حيلف الكراون على natural tooth وهني ال stress كله مرتكز علي جهة ال cervical اللي بيسبب stress هلبا على post and core ويؤدي إلى
"Post and core fracture "
⤵️⤵️
"Vertical root fracture and tooth loss"🤕

+ helps in promoting Hugging action between the tooth and artificial crown🤝
السن تعطي مقاومة أكثر للكراون والكراون يحميها 😌

+improves overall resistance of restoration✌

Factor that influence on ferrule effect?
شن الاشياء اللي منشوفهم ونقولو ره عندنا optimal
ferrule effect 🤔

1) ferrule height
طبعا كل ماكان الجزء اللي قاعد من السن أطول كلما كان أحسن .
طبعا اقل طول نقدر نحقق بيه ferrule effect هو
1.5mm
●نقيسوها عن طريق periodontal probe

والطول يعتبر أهم نقطة نركز عليها ⛔

2) ferrule weight
Thickness of wall should not be less than 1mm
اقل من واحد مليميتر يعتبر ضعيف هلبا
وميحققليش ferrule effect اللي نبيها

3) Number of wall and ferrule location
إن يكونو عندي 4wall هذا يعتبر optimal

لكن يكون عندي partial ferrule خير من مافيش 🤷‍♀️

يكون عندي buccal and lingual تعتبر good ferrule

نقطة اربعة وخمسة تأثر علي ferrule بشكل غير مباشر 🔁
4) type of the post
دارو دراسات ولقو إن :-
_Repairable fracture are seen with fiber post and 2 mm ferrule .
Fracture occurs more occlusally above cemento-enamel junction (CEJ)
AND
_Non-repairable fractures are seen with metal posts and 2 mm ferrule.
fracture occurs more Apically below CEJ.

لذلك قالو ان fiber post أفضل من metal لان

Fiber post created less stress on the root in apical and middle part.

5) type of core
الكومبوزت لان يرتبط مع اللي قاعد مع السن ويقويها أكثر يعتبر أفضل من الأملقم 👍

#Classification of the ferrule into categories is based on risk factor?

1)No anticipated risk
عندي هني 4wall intact+2mm height +1mm thickness
مافيش اي خطر علي السن ونقدر نلبسها واني مرتاحة 😌
2)low risk
عندي هني 3wall intact with mesial or distal missing+2mm height+1mm thickness
نقدر نلبسها وعادي تمشي امورها بإذن لله 😌

3)medium risk
2wall missing ➡️mesial and distal +2mm hight+1mm thickness
هني السن ممكن نعتبروها low risk لو متعرضتش لقوة كبيرة ^^
4)high risk
3wall intact with buccal or lingual missing+
2mm hight +1mm thickness
هني السن ممكن نعتبروها medium risk لو متعرضتش لقوة كبيرة ^^

أيضا في الhigh risk عندي Two wall intact اللي هما mesial and distal وعندي B and L مش موجودين .
وكان عندي two wall intact لكن في جانب بعضهم زي mesial و lingual أو buccal وdistal هني الخطر ان السن مش حتتحمل وتتعرض للكسر أكبر من ما لو كان مقابلين بعضهم.

وطبعا لو كان عندي one wall هني حتى هي تعتبر اكيد في high risk 😠

5) non-restorable ➡️➡️➡️➡️No ferrule 🚫

ووصلنا هني ونقولو إن السن من غير ferrule ابدا متتعوضش ✖ وفي من يخدم في كراون عادي ومعنداش ferrule بس هني علمياً خطأ والسن مش حتطول وتنكسر😤

●باهي في حالات نقدر نخلق ferrule واني معنديش clinical crown

زي مانعرفو انه عندنا⤵️
Anatomical crown sub gingiva
يمتد إلى Cemento enamel junction
ف شن حندير ؟
حاجتين نقدر نديرهم أولهم
1) Crown lengthening
A surgical procedure designed to the increased supragingival tooth structure.

ندير gingivoctomy باش نتحصل على
1.5mm of tooth structure subgingiva
ونحقق ferrule effect اللي نبيها 💪

2) orthodontic extrusion.
Orthodontic movement of tooth in occlusal direction.
عيبها ان بتاخد هلبا وقت بس .

وهكي نكون كملت😍 متنسوش تشوفو الصور 👆
