هذا السؤال اتكرر عليا اكثر من مرة
واجابتة بحسب كلام الدكتور محمد الدولة

بالنسبه للناس اللي تستفسر عن التعامل مع الاسنان اللي تجي وفيها قيح ومحتارين يخلوها مفتوحه

مسأله ترك السن مفتوح في حالة وجود قيح ام اغلاقه.......

هي مسئله خلافيه في الاندو ...
فهناك رأيين في هذا الموضوع

الرأي الاول ...
يقوم بترك السن مفتوح (اغلاق السن فقط ب قطنه) ويحتجون بذلك ان ترك السن مفتوح سوف يسمح للقيح بالخروج عن طريق القنوات وبالتالي يخف ألم المريض كثيييير جدا وكذلك الورم

اما الرأي الثاني ...
فيقوم باغلاق السن بحشوه مؤقته بعد الانتهاء من تنظيف القنوات...
ويحتجون بذلك ان ترك سن مفتوح سوف يسمح بدخول العديد من البكتيريا لداخل القناه و يتحول الـ infection
الى خليط من انواع البكتيريا وبالتالي تزداد مقاومتها للمعالجه.....

ماهو الحل الامثل ؟!!!
برأيي انا راح اقوم بالحل الذي يجمع بين إيجابيات الرأيين... ويجنبني سلبيات الرأيين

فاريد ان اقوم بعمل طريقه
تخفف لي الم المريض وكذلك الورم عن طريق اخراج القيح ( الرأي الاول)
وكذلك اغلق السن وعدم تركه مفتوح لتجنب تحويل الـ *infection*
من
*anaerobic* *infection*
الى
*mixed* *infection*

قبل شرح الطريقه...

سؤال يجب ان نجيب عليه

لماذا عندما يكون في هناك قيح وعند فتح سن يخرج القيح... وبعد الانتهاء من تنظيف وتوسيع القنوات يتوقف القيح من الخروج... ولكننا بمجرد ان نغلق السن بحشوه يعود لنا مريض خلال وقت اقل من 24 ساعه بالم شديد و ورم؟؟؟؟؟!!!!!

واذا تركناه مفتوح يخف الالم ويختفي الورم؟؟؟

عتقولو لاننا لما نغلق السن بنسد مجرى القيح وبالتالي بيزداد الالم ...

صحيح كلامك ..

ولكن هذا مجرد عامل وليس هو سبب الرئيسي...

السبب الرئيسي هو الذي اوجد القيح وكان السبب في تكونه...
الا وهو

ومش اي نوع انما النوع اللاهوائي

anaerobic bacteria

وزي محنا عارفين ان ال
anaerobic bacteria
تحب البيئه الخاليه من الاكسجين....

ويقل نشاطها في وجود الاكسجين....

ملاحظين فين الربط ف الموضوع....

فالبكتيريا اللاهوائية عندما تسبب القيح ونقوم نحن بترك السن مفتوح فان دخول الهواء راح يقلل من نشاطها كما انه يسمح بخروج القيح وبالتالي يلاحظ الطبيب تحسن ممتاز عند المريض... بينما عندما نغلق السن بحشوه فاننا نمنع دخول الهواء للقنوات وبالتالي يزداد نشاطها اكثر ويزداد الالم عند المريض

هل معنى الكلام هذا ان ترك السن مفتوح هو الحل الامثل....

بامكاننا ان نستخدم الهيدروجين بيروكسايد
hydrogen peroxide
ك ارقيشن ولي من خواصه انة يطلق جزيئات الاكسيجين
وبالتالي يعمل ع قتل البكتيريا اللاهوائيه الي كونت القيح....

الخطوات الي اقوم بعملها في حالة القيح...
1- بعد فتح القناه اعدي الابكس بفايل صغير 6 او 8 فايل
كي احفز خروج القيح (غالب الاحيان يخرج القيح بمجرد فتح السن مباشرة)

2- تحديد الاطوال والبدء في عمل

3- اعتمد فايل 35 او 40 ماستر ابكل فايل MAF كحد ادنى لكي يتم تنظيف الابكس جيدا وتوسيعه كي يسمح اكبر كم من القيح للخروج

4- استخدام ارقيشن
صوديوم هيبوكلورايد مركز
يتبعه
نرمل سلاين
يتبعه
هيدروجين بيروكسايد
يتبعه
نورمال سلاين

وهكذا تكرر العمليه بين كل فايل و فايل

5- عمل تنشيط للارقيشن... وهذا مهم جدا وفعااااااااااااااااال جدا.... واذا ماعملته ماراح تستفيد من الارقيشن بنسبه تصل الى 75%

6- عند الانتهاء من المعالجه
نقيم الوضع
فاذا كان القيح توقف ... ويتم تقييم ذلك باستخدام بيبر بوينت

اذا خرج من القناه و لونه اصفر يعني مازال القيح يخرج.... واذا لونه ابيض فمعناه توقف

في حال مازال القيح بيخرج
اقوم باستخدام سيرنجات الاندو ولي بتكون رفيعه جدا و اوصلها لحد الابكس .. ومن ثم اقوم بسحب السيرنج من اجل شفط القيح من منطقة الابكس ... وتكرار العمليه لحد توقف القيح...

اما اذا وقف القيح....
فقوم باغلاق السن بحشوه موقته
((حذاري )) ان تضع كالسيوم هيدروكسايد داخل القنوات لانه راح يزيد الحاله سوء
