ملحوظة مهمة جدا من كتاب "Easy endodontics" صفحة رقم ١٩٦ 👇
ملحوظة مهمة جدا وانت بتاخد working length 👇
فيه حالات الapex locator بيديك قرايه انك وصلت للapex وبعدين تيجى تصور x ray تلاقيك لسه بعيد عن radiographic apex فبتبقى محتار مين اللي صح هل تعمل preparation قصير زي مالapex locator بيقول ولا تزود الworking length وتمشى عل الx ray 🤔🤔
الحقيقه ان غالبا الapex locator هوا اللي بيبقى صح لان فيه 10% من الحالات الapical foramen بيبقى بعيد عن الanatomical apex of tooth بحوالى ٣ مللى فبتيجي تصور اشعه تلاقي الفايل بعيد عن الradiographic apex لكنه في الحقيقه واصل للapical foramen ولو زودت في working length عن كده هتعمل
over instruentation & over extended obturation 🙃😑

ال theoretical failure فى الإندو اللى هى باختصار فيه signs of failure موجودة لكن مفيش symptoms وده ممكن يحصل فى الحالات دى👇
✅ اول حالة انه يكون فيه problem فى ال endodontic procedures واضحه فى ال radiograph زى مثلا separated file او short obturation لكن مفيش symptoms 🤔
ال management هنا انك تعمل follow up للحالة لمدة كافية قبل ما تحط اى fixed prosthesis وقبل ما تقرر تعمل retreatment 👍
✅ تانى حالة ان يكون فيه periapical radiolucency تحت ال tooth على الرغم ان الإندو ظاهر فى ال radiograph انه كويس لكن مفيش symptoms 🤔
ال management هنا بردو انك تعمل follow up للحاله لمدة كافية قبل ما تعمل اى fixed prosthesis وقبل ما تفكر تعمل retreatment وتقارن ال radiographs تشوف هل بتقل كل فترة ولا لا
خلى بالك ان ال complete healing ممكن يحتاج فترة طويلة ممكن توصل ل ١٠ سنوات 👍
✅ تالت حالة ان يكون الإندو ظاهر انه كويس جدا فى ال radiograph ومفيش اى errors ومفيش اى periapical radiolucency ومفيش اى symptoms🤔 لكن ال coronal gutta percha كانت exposed لل oral cavity fluids لاكتر من ٣ شهور
ال management هنا انك لازم تعمل retreatment قبل ما تعمل اى final restoration 👍
