💊 هل يفقد الميتفورمين فعاليته مع مرور الوقت؟

يُعد Metformin العلاج الأول لمرض السكري من النوع الثاني، ويتميز بفعاليته العالية وسلامته وانخفاض خطر التسبب بنقص سكر الدم.

لكن من المهم معرفة أن بعض المرضى قد يفقدون السيطرة الجيدة على سكر الدم مع مرور الوقت، وهي ظاهرة تُعرف باسم Secondary Failure (الفشل الثانوي للعلاج).

📌 تشير الدراسات إلى أن معدل الفشل الثانوي للميتفورمين يبلغ حوالي 5–10% سنويًا.

أي أنه في كل عام، قد يحتاج 5–10% من المرضى الذين كانوا يستجيبون جيدًا للميتفورمين إلى إضافة دواء آخر أو تعديل الخطة العلاجية للحفاظ على التحكم في مستوى السكر.

⚠️ وهذا لا يعني أن الميتفورمين أصبح "غير فعال"، بل إن التطور الطبيعي لمرض السكري من النوع الثاني يؤدي إلى استمرار تراجع وظيفة خلايا بيتا (β-cells) في البنكرياس مع مرور الوقت، مما يستدعي تكثيف العلاج.

🔹 الخلاصة:
الفشل الثانوي مع الميتفورمين ليس بسبب حدوث "تحمل دوائي" (Drug Tolerance)، وإنما بسبب التقدم المستمر للمرض.
