✅عند معالجة ارتفاع البوتاسيوم يتم استخدام الأنسولين داخل محلول سكر dextrose لسببين بستخدم محلول السكر
اولا عشان dextrose بيعمل تحفيز ل الانسولين والانسولين بيعمل ازاحه للبتاسيوم من الدم لداخل الخلايا
ثانيا عشان اتجنب هبوط السكري الناتج عن استخدام الأنسولين في معالجة إرتفاع البوتاسيوم
✅ثانيا بستخدم محلول ملح في معالجة انخفاض البوتاسيوم الى جانب بوتاسيوم كلوريد ومابستخدم محلول سكر

Appendicitis & Antibiotics
دراسة على 856 مريض عندهم
CT-diagnosed uncomplicated acute appendicitis
اتعالجوا بـ antibiotics بدلاً عن العملية وجد:
إذا
appendiceal diameter ≥ 15 mm في CT
ومعها
body temperature > 38°C
فهؤلاء المرضى عندهم
higher risk of primary non-responsiveness
(يعني احتمال كبير الـ antibiotics ما تكفي ويحتاجوا surgery خلال 30 يوم).
CRP ≥ 100 mg/L
Leucocytes ≥ 9 × 10⁹/L
كانت
strongly associated
مع فشل العلاج بالمضاد.
بينما لو
CRP < 100 mg/L at 24 h →
عند المريض تقريبًا
99% likelihood of successful antibiotic therapy
prognostic factors
واضحة تقدر تتنبأ من بيرجع له
recurrent appendicitis
بعد نجاح العلاج بالمضادات.

🛑 metronidazole in pregnancy
✅ In general, use of metronidazole should be avoided, particularly during the first trimester
✅should only be used in pregnancy if treatment of infection is necessary and no safe alternative agents are available (eg, bacterial vaginosis, trichomoniasis)
✅(CDC) suggests that treatment of bacterial vaginosis with short courses of metronidazole (500 mg orally twice daily for seven days or 0.75 percent gel intravaginally daily for five days) in pregnancy appears to pose a low risk of teratogenicity or mutagenic effects.

✅The most plausible mechanism is decreased iron absorption due to H. pylori-associated gastric atrophy and hypochlorhydria
✅H. pylori infection can lead to chronic (metaplastic) atrophic gastritis resulting in hypochlorhydria and vitamin B12 malabsorption. Also, the bacteria may elicit production of antibodies that cross-react with the gastric parietal H+ K+ ATPase.

🛑ملاحظات بخصوص دواء Spironolactone و Eplerenone
✅تأثيرهما كاخفظ للضغط ضعيف لذا يعتبر خيار ثالث (ك اضافه ) في حاله uncomplicated HTN ولكنه يعتبر خيار اول في حاله resistance hypertension الذي يكون بسبب hyperaldosteronism
✅ بنسبه لEplerenone
في حاله استخدامة كعالج لارتفاع الضغط يفضل إعطاء الجرعه على مرتين في اليوم عشان يعطي فعاليه في خفض ضغط الدم ،ع عكس استخدامه في post MI او HF قد يعطي جرعه واحده في اليوم
