📚 Statins & Pregnancy | الستاتينات أثناء الحمل🤰

الدراسات الحيوانية 🧪📝 أظهرت أن statins قد تكون teratogenic⚠️ (أي تسبب تشوهات جنينية)، لكن الحقيقة في البشر ما زالت غير مؤكدة.❓

🔬 دراسة نرويجية شملت أكثر من 800,000 امرأة حامل🤰 وجدت أنه لا يوجد ارتباط بين استخدام الستاتينات في الثلث الأول من الحمل (first trimester) وحدوث تشوهات خلقية
(congenital malformations).

لذلك:
النساء اللاتي كنّ يستخدمن statins ثم اكتشفن الحمل يمكن طمأنتهن أن خطر التشوهات غير مرتفع.
لكن 😀 يُنصح بتجنب statins أثناء الحمل لأنها قد تزيد من خطر الإجهاض (spontaneous abortion) أو الولادة المبكرة (preterm labor).

📢 IV iron and Use of premedication
✅We do not give any premedications to patients without a history of asthma or more than one drug allergy

✅For patients with asthma or more than one drug allergy, who are at slightly increased risk of an allergic or infusion reaction, we routinely premedicate with 125 mg of methylprednisolone and an H2 blocker (eg, 20 mg of famotidine) given intravenously prior to administration of any IV iron product

✅For patients with a history of inflammatory arthritis, we administer methylprednisolone, 125 mg intravenously, and prescribe a short course of prednisone (1 mg/kg per day orally for four days

تشير دراسة بريطانية واسعة شملت أكثر من 1,000,000 حالة حمل إلى أن استخدام مضادات الاكتئاب خلال الثلث الأول من الحمل
(First Trimester)
لا يزيد من خطر الإجهاض بشكل مهم سريريًا.

📊 النتائج:
استخدمت 7.2% من النساء مضادات الاكتئاب خلال الثلث الأول.
نسبة الإجهاض كانت 14.7% بين المستخدمين مقابل 12.4% بين غير المستخدمين.
بعد ضبط العوامل المؤثرة (مثل القلق، التدخين، الحالة الصحية)، أصبح الارتباط ضعيفًا جدًا:
Adjusted HR = 1.04 (95% CI 1.02–1.06)
أي
13.6% vs 13.1%
— فرق بسيط وغير مهم إحصائيًا.

✅بنسبه ل مريض الفشل الكلوي المزمن يفضل تجنب مسكنات NSAIDs
✅الامريكان(Uptodate ) يقولوا ممكن استخدم مسكنات NSAIDs في مريض الفشل الكلوي المزمن لو في حاجه ملحه مثل osteoarthritis لو كان GFR فوق 45 وسوي monitoring كل ٣ اشهر لوظائف الكلى بينما الكندييين (Rxfiles ) قالوا ممكن نستخدمها بحذر لو حاجه ملحه لو GFR فوق 30 او قد المرض ماشي ع غسيل كلوي لانه ماعاد تفرق

✅بنسبه لمسكنات opioids يقول الامريكان(Uptodate ) لو كان GFR اكثر من 30ml استخدم نفس جرعه الشخص الطبيعي
وتجنب الترامادول وال morphine في حاله GFR اقل من 30
بينما الكندييين (Rxfiles ) قالوا ممكن استخدم الترامادول لو كان GFR اقل من 30 بجرعه يوميه لاتزيد عن 200mg ممكن نستخدم ال hydromorphin في حاله GFR اقل من 30
