Clopidogrel vs Aspirin — Which is better after CAD?

😀 ما أظهرته الدراسة الحديثة (The Lancet, 2025):
شملت قرابة 29,000 مريض 😵‍💫مع أمراض الشرايين التاجية.🫀💋
كلوبيدوجريل أحادي قلل من مجمّع الأحداث القلبية (الوفاة القلبية، احتشاء القلب، السكتة) بنسبة ~14٪🛍 مقارنةً بالأسبرين.
💊ولم يُسجَّل زيادة ذات دلالة في النزف الكبير بين المجموعتين.

😀 ماذا يعني هذا عمليًا؟

إذا انتهت فترة العلاج المزدوج (مثلاً أسبرين + مثبّط P2Y12) أو ما لم تبدأ بها، قد يكون كلوبيدوجريل خيارًا أحاديًا مفضلًا.

⚠️⚠️لكن أي تغيير في الممارسة يجب أن يأخذ بعين الاعتبار خصائص المريض الفردية تاريخ النزف تداخل الأدوية، تكلفة وتوفر الدواء وجود أدوية أخرى مثل مضادات التخثر، وجود عوامل جينية CYP219 عند الحاجة، وكذلك توصيات الدلائل الإقليمية /الوطنية.
