Vertex Pharmaceuticals
وحصل على موافقة الـ FDAفي يناير 2025 لعلاج الألم (pain).
يتميز بأنه أول مسكن من نوعه (first-in-class analgesic)
يعمل عبر تثبيط قناة الصوديوم العصبية Nav1.8،
وبالتالي يقدم خيارًا فعالًا (non-opioid)
لتخفيف الألم بعد العمليات الجراحية.
🔬 Mechanism of Action
Nav1.8 sodium channel
موجودة بشكل أساسي على
(small diameter sensory neurons).
هذه الأعصاب تنقل إشارات الألم (nociceptive signals)
إلى الجهاز العصبي المركزي.
يرتبط بأحد domains الخاصة باستشعار الجهد في القناة ويثبتها في الحالة المغلقة يمنع انتقال إشارات الألم.

✅Phenytoin is very unstable when diluted even in normal saline. Precipitation mayoccur which is not visible. It is recommended to administer solutionsimmediately upon preparation.

يعني كـ practice

الجراح أو طبيب التخدير يقدر يستخدمه كخيار بديل لتقليل اعتماد المرضى على
المورفين و الهيدروكودون
وبالتالي تقليل خطر الإدمان و الآثار الجانبية التنفسية

📌 مثلا في حالات
abdominoplasty أو bunionectomy

يمكن وصف Suzetrigine
بجرعة 100mg (loading dose)

ثم 50mg كل 12 ساعة

يكون كـ تسكين فعّال ومكافئ للأفيونات لكن بأمان أعلى.

🛑Pearls of Vitamin K (phytonadione)
✅1. Oral Vitamin K has similar efficacy as intravenous Vitamin K.

✅2. SQ (subcutaneous) Vitamin K absorption is unreliable.

✅3. IM (intramuscular) Vitamin K may promote intramuscular hemorrhage.

✅4. IV (intravenous) Vitamin K is effective in 6 - 8 hours

✅.5. IV Vitamin K should be given very slow (preferably .5 mg/min).

🛑PPI in treatment H.pylori
✅¶ PPIs should be taken 30 to 60 minutes prior to meals
🅰. Standard doses of oral PPIs for treatment of H. pylori infection in treatment-naïve individuals include lansoprazole 30 mg twice daily, omeprazole 20 mg twice daily, rabeprazole 20 mg twice daily, or esomeprazole 20 mg twice daily.
🅱Pantoprazole 40 mg twice daily is a nonpreferred option due to its weaker acid-inhibitory effect.

Musculoskeletal chest pain asked for vitamin D level

Severe Vitamin D deficieny

After correction of vitamin D deficiency:

No UTI
No worsening renal function
No chest pain
Significant improvement of fatigue
Decreasing dose of diuretics

I have searched in the literature and i found that severe vitamin D deficieny is significantly correlated to the risk of UTI!
