Hypertension resistance

Resistant hypertension is defined as the lack of blood pressure control when a patient is taking 3 antihypertensive medications with complementary mechanisms of action (with a diuretic as one component) or when blood pressure control is achieved but requires ≥4 antihypertensive medications

كيف يعمل
Endothelial receptor antagonist; inhibits binding of endothelin (ET)-1 to ETA and ETg receptors.

طيب ماذا يعمل ال endothelial

Endothelin-1 causes vasoconstriction, fibrosis, cell proliferation, and inflammation via the ETA and ETB receptors.
Activation of ET-1 in hypertension can cause endothelial dysfunction, vascular hypertrophy and remodeling, sympathetic activation, and increased aldosterone synthesis.

🧬 CAR T-cell Therapy–Associated HLH (CAR-HLH) Management

وفقًا لتوصيات EBMT Guidelines (2025) لإدارة متلازمة Hemophagocytic Lymphohistiocytosis (HLH) المرتبطة بـ CAR T-cell therapy:

🔹 Early CAR-HLH (مع refractory CRS):

First-line: Anakinra بجرعة 1 mg/kg IV q6h + Dexamethasone 10 mg IV 2–4 مرات يوميًا.

Second-line: إذا لم يتحسن المريض أو ارتفع Ferritin، تعطى جرعة ثابتة Anakinra 100 mg IV q6h.

Third-line: في حال التدهور، يستخدم Etoposide 50–75 mg/m² IV (يوم 1، مع إمكانية إعادة الجرعة في يوم 8).


🔹 Late CAR-HLH (بعد تحسّن CRS أو انحساره):

First-line: Anakinra 1 mg/kg IV q6h ± Dexamethasone 10 mg IV 2–4 مرات يوميًا.

Second-line: إذا لم يحدث تحسن، إعطاء Anakinra 100 mg IV q6h + Dexamethasone (إذا لم يُستخدم أولاً) أو التفكير باستخدام Ruxolitinib.

Third-line: إذا استمر التدهور، يُستخدم Ruxolitinib (إذا لم يبدأ سابقًا) أو Etoposide 50–75 mg/m² IV (يوم 1، ويُعاد يوم 8 عند الحاجة).


📌 Clinical Insight:
النهج العلاجي يعتمد على التصعيد التدريجي (Stepwise escalation) باستخدام Anakinra → Steroids → Ruxolitinib/Etoposide حسب مرحلة CAR-HLH واستجابة المريض.
