في أمراض السكر من النوع الثاني من المعروف أنة كان الخيار الأول هو ال metformin لكن حسب الجمعية الأوربية والأمريكي لم يعد مفضل ك خيار أول لكل المرضى ف يعتمد الخيار الأول على حسب المريض
في حاله أنه لم نحقق ال target A1c وهذا يختلف من شخص إلى آخر
أو كان عند المريض comorbidities
(eg, ASCVD, heart failure, CKD, obesity).
في هذه الحالة نختار ع حسب
patient- and drug-related factors
أما بالنسبة للمريض مذكور فوق أما بالنسبة لدواء
Drug-specific factors to consider include glucose-lowering efficacy, impact on other comorbidities, impact on weight and hypoglycemia risk, side effect profile, ease of use, and cost

في حالة كان يعاني من
high risk or established ASCVD,
هنا نفضل نختار ي أما
SGLT2i or GLP-1 RAs
والتي تمتلك
proven CV benefit (canagliflozin, empagliflozin, dulaglutide, liraglutide, or SC semaglutide)
أما أذا كان عندة فشل قلبي HF
هنا نفضل
(empagliflozin, canagliflozin, dapagliflozin)
أما أذا كان CKD نفضل
(canagliflozin, dapagliflozin)
لانه تقلل من تدهور الكلى

أذا لم يحقق ال target من إستخدام دوائين بعد ثلاثه أشهر هنا نضيف الدواء الثالث على حسب.

لكن في حاله أنه لا يعاني من هذه الأمراض ولكن كان يعاني من زيادة الوزن
