بنسبه لمده تناول Vancomycin infusion بيعتمد على الجرعه
✅اذا كانت الجرعه اقل من 1 جرام ناخذها على مدار ساعه
✅الجرعه بين 1 جرام إلى 1.5 جرام ناخذها على مدار ساعه ونصف
✅الجرعه فوق 1.5 ناخذها على مدار ساعتين

✅كما هو معروف بأن مانع الحمل المركب(المحتوي على استروجين +بروجتسيرون ) ممنوع في النساء فوق عمر 35 عام اذا كانت المرأه تتعاطي السجائر ولكن هل يتوقف الأمر على عدد السجائر؟

✅الاجابه نعم اذا كانت تاخذ بما يعادل ١٥ حبه سجائر او اكثر في اليوم هنا يمنع تاخذ مانع الحمل المركب

دراسه كانت مقارنة بي إستخدام
Atorvastatin ve rosuvastatin
Among patients with coronary artery disease,

كانت خلاصته انه
rosuvastatin was not superior to atorvastatin at preventing adverse cardiac events within 3 years.

كان هذا النقص في الفايدة برغم ان دواء ال rosuvastatin عمل ع تنقيص ال.
LDL-C level <70 mg/dL at 3 years
في تلك المجموعه.
لكن
Rosuvastatin was associated with an excess incidence of initiation of diabetic medications and cataract operations.

ع حسب هذه الدراسه فإن

high-intensity atorvastatin therapy may be preferential to rosuvastatin.

الخلاصة انه يفضل إستخدام high-intensity atorvastatin
على إستخدام ال
rosuvastatin
والسبب انه تكون رسك لمرض السكر

ليش هذه فقط ؟
ومتى افضل كل واحد ؟
وهل هناك غيرهن ؟

اولاً ليش هذه فقط
السبب يعود انه تمت عدة دراسات ع الأدوية التي تقلل من الأمراضيه والوفيات فكانت هذه هي
بينما البقية وجدوا أنها ي أما إنها لا تقلل او أنها تعمل ع زيادة الأمر سوئاً

أما بالنسبة ل متى أختار كل واحد وأيهنا أفضل  يعتمد ع المريض والحالة

بالنسبة الأفضلية كانت لل Carvedilol
ع شان نعرف السبب دعونا أولاً نعلم كيف يشتغل
الدواء هذا يعتبر
carvedilol is a third-generation nonselective β-blocker that competitively blocks β1-, β2-, and α-receptors with some antioxidant properties.
وطبيعة الحال
Approximately 80% of adrenergic receptors in the normal myocardium are β1-receptors.
However, due to β1 downregulation in HF, β2 may rise up to 40% of the total adrenergic receptors.
لذالك هذا ميزة لدواء 
وأيضاً بما أن ال
carvedilol blockade of α1-receptors causes vasodilation of blood vessels and might help to improve afterload
لكن هذا قد يكون
higher risk for hypotension.
أيضآ من جانب آخر وجد في دراسه أن ال carvedilol أكثر واحد في تقليل الوفيات

طيب هذا الكلام لا يعني أن البقية ليس لهم منفعة
بالنسبه لدوائين الآخرين يعتبرين

وقد كان ال
Metoprolol is 75-fold selective, whereas bisoprolol has approximately 120-fold
ف هذه نفضلها في حالة انه هناك مانع لاستخدام الدواء الأول مثل الربو وأمراض السكر
او في حالة ال HR أكثر من ال BP


هل هناك دواء اخر ؟
نعم هناك دواء اسمه
Nebivolol
ويعتبر
  selective β1-receptor antagonist
ولة خاصية
vasodilatory
والسبب يعود
  NO production via activation of β3-adrenergic receptors.
وهذا هو أكثر دواء انتقائي ع  β1

It is approximately 3.5 times more β1-selective than bisoprolol
لكن وجد انه لا يقلل من الوفيات والأمراضية إلا في كبار السن الذي أعمارهم  70 سنة او أكثر
لكن غير مصرح من ال FDA فقط من الجانب الأوربي.

🛑In patient with acute-on-chronic liver failure (ACLF)


⚫ Critically ill patients with ACLF and any form of upper gastrointestinal bleeding should be treated with prophylactic antibiotics, usually third-generation cephalosporins

⚫ Critically ill patients with ACLF and portal hypertensive bleeding should be treated with octreotide or a somatostatin analog (moderate quality evidence) and a proton pump inhibitor (low-quality evidence).
✅Critically ill patients with ACLF and SBP should receive broad-spectrum empirical antibiotics, including consideration of coverage for resistant organisms.
⚫ Patients with ACI.Fand SBP also should receive albumin, even if they are not clearly volume depleted.
(as moderate-quality evidence shows that albumin lowers risks for acute kidney injury and death)
