الجديد انه ممكن نستخدم ادويه SGLT2 inhibitors في مريض ACS اذا تصاحب مع السمنه😁😁

قبل كان نتجنب Fondaparinux اذا كان Crcl اقل من 30 الان اذا كان اقل من 20

في التحديث الأخير الجايد لاين الأوربي لأمراض الفشل القلبي
كانت أنه اذا كان مريض T2DM and CKD

نستخدم أدوية SGLT2 inhibitors (dapagliflozin or empagliflozin) وا finerenone
السبب ع شان نقلل من ال
to reduce the risk of HF hospitalization.
لكن ما معنى
CKD !?

CKD was defined as follows:

eGFR 25–75 mL/min/1.73 m2
and a urinary albumin-to-creatinine ratio ≥200–5000 mg/g (ACR)
حسب دراسة ال outcomes from dapagliflozin
DAPA-CKD;

او يكون ال eGFR
20–45 mL/min/ 1.73 m2
او يكون
eGFR 45–90 with (ACR) ≥200 mg/g
حسب دراسة EMPA-KIDNEY

او يكون
eGFR 25–60 mL/, a (ACR) 30–300 mg/g, and diabetic retinopathy,
او
eGFR 25–75 mL and (ACR) 300–5000 mg/g, in

.For patients with IE and stroke, thrombolytic therapy is not recommended However, thrombectomy may be considered in selected cases with large vessel occlusion.

✅بالعربي في حالة ischemic stroke ممكن نستخدم مذيبات الجلطة thrombolytic في المعالجة تحديد اول اربع ساعات ونصف من ظهور الاعراض لكن لاتستخدم مذيبات الجلطة اذا كان مريض ischemic stroke مترافق مع infective endocarditis 😁

Effects of corticosteroids on severe community-acquired pneumonia:
وجدوا ثلاثه اشياء
الاول
We found that hydrocortisone was
associated with a reduction in all-cause mortality but this observation was not seen for non-hydrocortisone corticosteroids
ثانيا كانت النتائج ان المرضى الذي تلقوا
corticosteroids, particularly hydrocortisone,
لمدة 8 ايام او اقل
without tapering,
experienced significantly lower mortality risks
,‌‌
وكنت نفس النتائج عند من استخدم
hydrocortisone,
جرعه
200mg with a gradual taper over 8 or 14 days
لذالك ف أن الأفضل استخدامه 8 ايام او اقل
الاستنتاج الثالث لم يتكلم عنه المؤلفين
عن خطر ارتفاع السكر في الدم الناتج من إستخدام ال corticosteroids
حيث أجريت تحاليل وجد أن هناك زيادة ب نسبة 50% تقريبا في مقارنة بال placebo
ف يجيب الحذر خاصة عند مرضى السكر

🛑بخصوص ملخص تحديثات الجايدلاين الاوروبي في معالجة ACS كالاتي

✅Among new medical therapies, sodium–glucose cotransporter-2 (SGLT-2) inhibitors receive class I recommendations in patients with type 2 diabetes or left ventricular systolic dysfunction.
✅Colchicine gets a mention as a IIb (weak) recommendation.
✅Beta-blockers are now a class III (no benefit) recommendation if there is no recent myocardial infarction, systolic dysfunction, or angina.

✅In patients who receive long-term anticoagulation, guidelines recommend dual antiplatelet therapy for only 1 to 4 weeks, followed by clopidogrel for 6 months.

وجب التنبيه عند استخدام دواء الاكتئاب duloxetine بأنه قد يزيد من خطوره النزيف اذا استخدم مع الأسبرين وايضا قد يساهم في ارتفاع ضغط الدم خاصه اذا كان غير متحكم به ⬅️هذه المخاطر احتمالية حصولها بيكون بشكل اكبر في مرضى كبار السن(فوق 65 عام)
