🉐Statin pretreatment in patients with acute coronary syndromes is associated with improved clinical outcome. However, discontinuation of statins after onset of symptoms completely abrogates this beneficial effect.

🉐Those who receive statins then stop it have a worse prognosis than those who did not receive statin from the start (rebound increase in CRP)

كما هو معروف بأن ادوية Statin ممنوعه في النساء الحوامل ولكن هناك بعض الدراسات موخرا قالت بأنه قد ناخذ بعين الاعتبار استخدام هذه الادوية في النساء الحوامل اللواتي very high risk of cardiovascular disease وحط الف خط تحت كلمه very high مثل
✅homozygous familial hypetcholserolemi
✅established cardiovascular disease

🛑بنسبه لإيقاف هذه الادوية قبل الحمل على الاقل ٦ اسابيع قبل التخطيط للحمل ويفضل 12 اسبوع

DOSE trial:
Among patients with acute decompensated HF, high-dose loop diuretics are associated with better symptom improvement than low-dose loop at the cost of some renal impairment, while continuous diuretic infusions are no better than intermittent diuretic boluses.

✅طبعا بخصوص Beers criteria  المختص بالمعالجة في كبار السن ( فوق عمر 65 سنة) هناك تحديثات جديده نزل في شهر 7 للعام 2023 وهي كالاتي

✅Avoid use of rivaroxaban for long-term treatment of nonvalvular atrial fibrillation or venous thromboembolism (VTE), due to higher risk of bleeding in older adults than other direct-acting oral anticoagulants (DOACs).

✅Avoid warfarin as initial therapy for VTE or nonvalvular atrial fibrillation unless alternatives (eg, DOACs) are contraindicated or there are substantial barriers to their use (eg, formulary restrictions)
✅
Avoid sulfonylureas as first- or second-line monotherapy or add on-therapy due to higher risk of cardiovascular events, all-cause mortality, and hypoglycemia than alternative choices. If a sulfonylurea must be used, then a short-acting agent is preferred.

✅Avoid initiating aspirin for primary prevention of cardiovascular disease. Consider deprescribing aspirin in older patients already taking it for primary prevention.

✅طبعا لايوجد اتفاق واضح بين اكثر  من جايدلاين  حول موعد  البداء او إعادة ادوية مضادات التجلط في مريض ischemic stroke  والمريض الذي  بحاجه لادوية مضادات التجلط سوى مريض الرجفان الاذيني او غيره ولكن هناك اقتراحات وتوصيات مختلفه بناء على إجماع الخبراء ( مافيش دراست سريرية معينه) ان هذا الأمر يعتمد على حجم الخثره ووجود risk ل التحول إلى hemorrhagic stroke. او وجود ارتفاع في ضغط الدم غير متحكم فيه  فكان الرأي  حسب الحاله كالاتي
✅TIA – For patients with a transient ischemic attack (TIA) and atrial fibrillation, oral anticoagulation can be started immediately.
✅Small- to moderate-sized infarct – For medically stable patients with a small- or moderate-sized infarct, warfarin can be initiated or restarted soon (eg, 24 hours) after admission with minimal risk of transformation to hemorrhagic stroke. We prefer to wait for 48 hours to start a DOAC, as DOACs have a more rapid anticoagulant effect.
✅Large infarct – For patients with large infarctions, symptomatic hemorrhagic transformation, or poorly controlled hypertension, withholding oral anticoagulation for one to two weeks is generally recommended
