الاستبيان بسيط جدا ،،، لا يحتاج اكثر من ٥ دقائق لتعبئة

حيث أن هوية جميع المشاركين في هذه الدراسة ستبقى مجهولة و جميع الرّدود على الاستبيان ستعامل بسرية .

🛑متى نستخدم دواء Bromocriptine في معالجة مرض فشل عضلة القلب؟

✅الإجابة في حاله Peripartum cardiomyopathy ،ماذا يعني هذا المرض وكيف يحدث ؟
✅الإجابة هو نوع من انوع الفشل القلبي النادر الذي يحدث للمرأة الحامل  اخر شهر من الحمل او اول خمسه اشهر بعد الولاده 

✅يتسم بأنه EF يكون أقل من 45% ويرجع البعض  السبب ل انحلال هرمون البرولاكتين وهذا بدوره يعمل myocardial toxicity  وهذا يفسر تحسن الحالة نتيجه استخدام anti prolactin(دواء Bromocriptine )


🛑هناك جرعتين لهذا الدواء حسب الحاله
✅اذا كانت الحاله uncomplicated يستخدم ال Bromocriptine بجرعه 2.5 ملجم في اليوم لمده اسبوع
✅اذا ترافق مع الحالة cardiogenic shock او كان EF اقل من 25% هنا نستخدم دواء Bromocriptine بجرعه 2.5 ملجم مرتين في اليوم لمده اسبوعين ثم 2.5 ملجم في اليوم لمده سته اسابيع
✅يتوجب إعطاء parenteral anticoagulation عند اخذ دواء Bromocriptine في هذه الحالة لانه فرصه الخثرات بتكون مرتفعه
✅المرأه التي التي لم يرتفع EF فوق 50% بعد المعالجة يجب أن تمنع من الحمل مره اخرى

13-Any wide complex tachycardia in patient with ischemic heart disease should be managed as VT until proved other wise

🛑anticoagulants in treatment ACS (NSTEMI)
✅In patients on oral anticoagulation, PCI performed without interruption of vitamin K antagonist or NOACS.

✅In patients on Vitamin K antagonist, do not administer Unfractionated heparin (UFH) if INR value 2.5.

✅-In patients on NOACs, regardless of the timing of the last administration of NOACs add additional low-dose parenteral anticoagulation (e.g. enoxaparin 0.5 mg/kg IV or UFH 60 IU/kg

✅بالعربي كذا بنسبه للشخص الذي ماشي ع مضادات التجلط لأي سبب ودخل في نوبه احتشاء قلب حاده واحتاج لعمل قسطرة قلبية لايتم ايقاف مضادات التجلط  التي ماشي عليها
✅اذا كان ماشي ع دواء Warfarin وكان INR فوق او يساوي 2.5 هنا ممكن نهمل جرعه الهيبارين المستخدمه في معالجة نوبه الاحتشاء
✅اما لو كان ماشي ع ادوية NOAC هنا ممكن اعطي الهيبارين او الكليكسان بجرعه صغيره تقريبا نصف الجرعه العلاجية في معالجه نوبة الاحتتشاء

🉐Statin pretreatment in patients with acute coronary syndromes is associated with improved clinical outcome. However, discontinuation of statins after onset of symptoms completely abrogates this beneficial effect.

🉐Those who receive statins then stop it have a worse prognosis than those who did not receive statin from the start (rebound increase in CRP)

كما هو معروف بأن ادوية Statin ممنوعه في النساء الحوامل ولكن هناك بعض الدراسات موخرا قالت بأنه قد ناخذ بعين الاعتبار استخدام هذه الادوية في النساء الحوامل اللواتي very high risk of cardiovascular disease وحط الف خط تحت كلمه very high مثل
✅homozygous familial hypetcholserolemi
✅established cardiovascular disease

🛑بنسبه لإيقاف هذه الادوية قبل الحمل على الاقل ٦ اسابيع قبل التخطيط للحمل ويفضل 12 اسبوع
