بنسبه ل دواء Furosemide في CHF في الشخص الذي لم ياخذ مدرات من قبل ممكن نبدأ ب 20 إلى 40 ملجم في اليوم مره او مرتين في اليوم ،في حاله لم يكن هناك استجابة نفضل رفع الجرعه مره واحده في اليوم بدل من اخذ الجرعه على مرتين في اليوم

✅بالعربي كذا بنسبه لادوية الضغط في مريض الذئبة الحمراء يفضل ادوية ACEI لأنها تقلل من cardiovascular disease

🛑لماذا لانعطي حقن لازكس عند ارتفاع ضغط الدم الشديد ،وهل هناك حالات تستدعي ذلك

السبب كالآتي
Lasix is mainly a venodilator
drug...with very weak arterio-dilator effect.
- hypertention is a disease of vasoconstriction of arteries and arterioles
صح راح ينزل الضغط بس

بعد ربع ساعه  بتشوف للضغط راجع عالي مره ثانيه
وطبعا لان لازكس يعتبر poten diuretic
بيخلي المريض يتبول بسرعه وبكميه كبيره وبكذا راح تقلل بشكل مفاجى  السوائل داخل intervascular  يعني تقلل من الضغط بشكل مفاجئ داخل large and medium sized arteries

وهذا بيخلي arteries تفكر بأنه حجم وضغط السوائل داخلها قل فجاءه بسبب نزيف مش تبول
وهذا بيخلي compensatory mechanisms يشتغل ويحصل  sever vasoconstriction  يعني ارتفاع شديد ومفاجى  للضغط
يعني ممكن يحصل resistance hypertension

ممكن استخدم Lasix في ارتفاع الضغط الشديد اذا كان مصاحب ل إحدى الحالات الآتية

✅pulmonary oedema
✅generalized oedema
✅renal failure with congested

#تذكر
قد نجد ادويه مضادات التجلط NOAC تستخدم في اغلب حالات pulmonary embolism لكن لايوجد لها أي استخدام في الحالات التالية

✅Patients with antiphospholipid syndrome
✅ chronic thromboembolic pulmonary hypertension (CTEPH)
✅ pregnancy or lactation
