كلام البورد الأمريكي في القلب ACCP2022 بخصوص مسكنات NSAIDs و Cardiovascular risk
الذي لفت انتباهي انه ال risk ممكن خلال اول اسبوع من الاستخدام

ادوية السكري من عائله Sulfonylureas قد تسبب ارتفاع في الوزن مثل glyburide and glipizide وقد لاترفع الوزن او تنقص منه مثل دواء Glimepiride

🛑فائده إنقاص ضغط الدم المرتفع باستخدام الادوية كالاتي
✅خفض معدل الجلطات الدماغيه بمعدل 35_40%
✅خفض معدل اختشاء عضله القلب بمقدار 20_25%
✅خفض معدل فشل عضلة القلب بنسبه تصل إلى اكثر من 50%

Intravenous iron during pregnancy (May 2023)

✅Oral iron is generally used to treat iron deficiency in pregnancy, especially during the first trimester. However, intravenous (IV) iron may be indicated in some cases and has the advantage of providing the entire dose in a single infusion. In a new open-label trial from Malawi that randomly assigned 862 pregnant individuals at a median gestational age of 22 weeks with hemoglobin <10 g/dL to receive IV or oral iron, the IV iron group had a lower incidence of iron deficiency at all time points and a lower incidence of anemia that reached statistical significance at some time points 
IV iron is especially useful for severe anemia and later in pregnancy when rapid repletion is needed.

✅بالعربي كذا المرأه الحامل التي تستخدم ادوية البيتا بلوكر لأي سبب كان ( فشل قلبي ؛شقيقه) عند الولاده يجب مراقبة المولود لمده اربع ايام بعد الولاده لانه ممكن يحصل هبوط في السكري او النبض او مشاكل في التنفس

بنسبه لادوية البيتا بلوكر المستخدمه في معالجه الفشل القلبي للمرأة الحامل نتعامل معاها مثل اي مريض ممكن نستخدم إحدى الثلاثه الادوية
✅Carvedilol
✅metoprolol
✅bisoprolol
ولكن نفضل ال metoprolol لانه اقل دواء يتعارض مع مستقبلات بيتا 2 المسؤله عن ارتخاء عضله الرحم اضافه إلى انه اقل احداث ل peripheral vasodilation.

🛑كيف نعالج النوبه الحاده للنقرس في مريض بيأخذ مضادات التجلط؟
✅يمكن نستخدم ال colchicine لانه لاياثر على  blood clotting.
✅في حالة كان هناك مانع لاستخدام colchicine يمكن استخدام oral glucocorticoids وليس الحقن وتستخدم بحذر مع مراقبه في حال كان هناك history of peptic ulcer disease or gastrointestinal bleeding.
✅When oral glucocorticoids are contraindicated, a highly experienced clinician can safely inject glucocorticoids into the joint of a patient on anticoagulation.


✅عندما يكون هناك مانع لاستخدام both colchicine and glucocorticoids ممكن نستخدم celecoxib لانه ليس لها antiplatelet effect 
✅عندما يكون هناك مانع لاستخدام كل من glucocorticoids, NSAIDs و , colchicine ممكن نتسخدم anakinra

تقريبا ادوية Glucocorticoids بجرعه معقوله مناسبه للاستخدام في المرضعات ولكن في حاله استخدام الام المرضعة دواء prednisolone بجرعه 20 ملجم او اكثر ينصح بتخلص من الحليب اول اربع ساعات من تناول الدواء

 There is limited information on the safety of most NSAIDs in breastfeeding women
✅. Ibuprofen is the preferred NSAID in breastfeeding individuals but only because it is secreted into breast milk in very small amounts, and there is the most information on its safety.
Ibuprofen levels were undetectable in the breast milk of individuals taking 1600 mg of ibuprofen a day

بالعربي كذا بخصوص مسكنات NSAIDs في المرضعات في المجمل لايوجد معلومات كافيه بخصوص مدى safety ولكن نفضل دواء ibuprofen لانه لم يلاحظ افرازه في الحليب حتى جرعه 1600ملجم في اليوم

أهمية استخدام lisinopril في مريض السكري بعد نوبه احتشاء القلب الحاد acute MI
✅انخفاض كبير بنسبة 29.8 في المائة في معدل الوفيات لمدة ستة أسابيع بين مرضى السكري بعد احتشاء عضلة القلب
✅إنقاض حياه 37 شخص من أصل 1000 مريض

لكن نجد الارقام هذه قليله اذا كان المريض غير مصاب بالسكري بحيث يقلل من معدل الوفيات بنسبه 5% وانقاض حياة 3 مرضى من 1000 مريض

GISSI-3 trial enrolled 18,131 patients of whom 2790 were diabetics. Treatment with lisinopril was associated with a significant 29.8 percent reduction in the six-week mortality among diabetics after myocardial infarction (MI), translating into 37 lives saved per 1000 patients. Lisinopril was less effective in nondiabetics; there was only a 5 percent reduction in mortality or three lives saved per 1000 patients.
