يعني في حالة عدم وجود مقاومة للمدرات والتي غالبا ترتبط ب Furosemide لايوجد فرق انه اعطي torsemide او Furosemide فمويا بعد الخروج من المستشفى لمريض مابعد الفشل القلبي الحاد

كما هو معروف بأن clindamycin هو خيار مناسب في حاله GAS المترافقة مع toxic shock syndrome
لكن ال resistance ل clindamycin في تزايد قد يكون linezolid هو خيار بديل في الأيام القادمة في انتظر دراسة تؤكد ذلك

🛑كيف يتم معالجة الصرع في النساء الحوامل؟

✅اغلب ادوية الصرع غير آمنة كليا في النساء الحوامل وقد تسبب تشوهات لدى الجنين مثل

✅neural tube defect (valproate, carbamazepine)
- ✅congenital heart defects (phenobarbitals, phenytoin)
- ✅orofacial clefts (carbamazepine, phenytoin)
🉐ولكن عدم معالجة الصرع للأم قد يأثر أيضا على الجنين

🛑generalised convulsive seizures can be harmful to the fetus due to trauma, alterations in electrolytes, blood pressure, and oxygenation

🛑Repeated generalised convulsive seizures during pregnancy have been shown to be associated with a higher preterm birth risk, and a reduced birth weight in boys

✅فرصه انه يحصل تشوهات للجنين من ادوية الصرع قد تصل من 4_10% ولكن هذا يعتمد على نوعيه الدواء والجرعه مثلا بإمكان التقليل من ذلك من خلال اخذ جرعه دواء اقل مثل
✅carbamazepine <400mg/day
✅Lamotrigine< 300mg/day
✅Levetiracetam as monotherapy

وتأخذ
✅Folic acid 5 mg/day
من قبل الحمل حتى نهاية الحمل

تجنب استخدام دواء Sodium valproate❌❌

فائده tranexamic acid في نزيف بعد الولاده او Trauma بيكون افضل كلما أعطيت بشكل اسرع وتقل الفائده كلما مر الوقت كل ربع ساعه تمر كلما قلت الفائده بنسبه 10%

❎ the survival benefit decreasing by about 10% for every 15 minutes of treatment delay until 3 hours, after which there was no benefit

🛑لماذا نعطي corticosteroid في مرضى septic shock ومتى نعطيها بضبط؟

✅بالمختصر في مريض sever septic shock قد يصاحبه corticosteroid insufficiency
ويمكن معرفه ذلك من خلال فحص adrenocorticotrophic hormone stimulation test ، أو يمكن معرفه ذلك من خلال ان المريض احتاج الى جرعه كبيره من vasopressor ولم يصل إلى target MAP
بتحديد جرعه epinephrine or norepinephrine] ≥0.25 mcg/kg/min for at least 4 hours after initiation to maintain the target MAP).

✅بنسبه لمعالجة Hepatorenal syndrome يعتبر زراعة الكبد هو العلاج الوحيد الامثل
ولكن قد نستخدم ادوية
مثل terlipressin ولكن قد ممكن يعمل respiratory failure خاصه في المرضى الذين عندهم respiratory comorbidities
حسب الدراسه التي نشرت في مجلة NJEM عام 2021

يتم اضافه albumin أيضا
