✅ال ESC 2021 guidelines يجيب بانة نفضل single pill combination لعده اسباب

✅Initial combination therapy (even low-dose combination) is more
effective & faster at lowering BP than monotherapy.
يعني خفض ضغط الدم بيكون اسرع

2.✅ Lower risk of adverse effects.
بتجنب الاعراض الجانبيه للدوائين نتيجة رفع الجرعه

✅Overcome treatment inertia (where patients remain on one drug long term despite inadequate BP control).

يعني اني اوصل إلى target goal بأسرع وقت😁

🛑ماعدا في بعض الحالات مثل

✅ low-risk, grade 1 hypertension (particularly if SBP <150 mmHg),

✅ frail older patients (age >80 years

موضوع موعد إعطاء vassopressors بعد fluid في مريض septic shock عليه كلام كثير من قبل بعض الدراسات يقول خلال اول ساعه وبعض الدراسات قالت ساعتين والبعض قال خلال ٦ ساعات

المهم في درسة جديده قارنت بين
✅restrictive fluid strategy(prioritizing vasopressors and lower intravenous fluid volumes
يعني بأنه نعطي كمية قليله من fluid مع إعطاء أولوية للبدا مبكرا ب vassopressors قارنت مع
✅liberal fluid strategy (prioritizing higher volumes of intravenous fluids before vasopressor use) for 24 h
يعني بأن اعطي كمية كبيرة من fluids قبل إعطاء vassopressors

التقيم في المجموعتين كان بعد 4 ساعات من إعطاء fluid (واخذو من 1 إلى 3 لتر) وكان في refractory hypotension

ال Outcomes كان
✅ primary outcome was death from any cause before discharge home by day 90

النتيجه كان مافيش فرق بين المجموعتين من حيث ال morality

✅ Among patients with sepsis-induced hypotension, the restrictive fluid strategy that was used in this trial did not result in significantly lower (or higher) mortality before discharge home by day 90 than the liberal fluid strateg

For patients with Chronic Cornary Syndrome(CCS) and diabetes, the target HbA1c level is < 7.0%
وايضا الادويه الي نفضله
• Metformin, TZD, GLP-1 receptor agonists, and SGLT2 inhibitors
ولكن

In patients with CCS and a history of ischemic stroke, GLP-1 receptor agonists are more effective than SGLT2 inhibitors

• In patients with CCS and a history of HF or CKD, SGLT2 inhibitors are the preferred medications

يعني في حالة عدم وجود مقاومة للمدرات والتي غالبا ترتبط ب Furosemide لايوجد فرق انه اعطي torsemide او Furosemide فمويا بعد الخروج من المستشفى لمريض مابعد الفشل القلبي الحاد

كما هو معروف بأن clindamycin هو خيار مناسب في حاله GAS المترافقة مع toxic shock syndrome
لكن ال resistance ل clindamycin في تزايد قد يكون linezolid هو خيار بديل في الأيام القادمة في انتظر دراسة تؤكد ذلك
