متى نستخدم الانسولين في أمراض T2DM بالنسبه لتراكمي
اذا كان التراكمي فوق 10 ف نفضل إستخدام
basal insulin
وبجرعه
0.1-0.2U/kg

او اذا كان عنده استخدامه اخر مثلا السكر فوق 300 مع اعراض او ماشي ع دوائين او اكثر وما تحسن
طيب في هذه الحاله
نستخدم له انسولين ولكن الجرعه تختلف ع حسب التراكمي

اذا كان نريد نستخدم
Long-acting insulin
اذا كان التراكمي اقل من 8 نعطيه بجرعه
0.1-0.2
واذا كان اكثر من 8 نعطي بجرعة
0.2-0.3

وكيف نعمل معايرة ؟
نعمل كل ثاني او ثالث يوم معايرة
يا اما نرفع الجرعه بمقدار وحدتين من الجرعه الكلية حتى نصل إلى الهدف المطلوب
او نرفع ع حسب السكر الصايم
اذا كلن اكثر من 180
نرفع بمقدار 20% من الجرعة اليوميه

ماعداها نفضل ادوية NOAC

🛑هناك ملاحظه مهمه بخصوص إعطاء دواء metolazone إلى جانب IV loop diuretics في معالجة refractory edema بأنه يتوجب بأن نعطي oral metolazone قبل IV loop diuretics بساعتين الى خمس ساعات
لانه بحتاج إلى ساعتين إلى خمس ساعات عشان ال metolazone يوصل إلى peak effect

🛑ملاحظات بخصوص loop diuretics
✅يكون هناك فائده في الفعالية عند تحويل ال Furosemide الفموي إلى نوع آخر من loop diuretics مثل torsemide الفموي لكن لايوجد فائده في الفعالية عند تحويل ال Furosemide الوريدي إلى torsemide الوريدي

✅في حالة عدم استجاب المريض ل maximum does من IV loop diuretics لن يستجيب لنوع اخر من المدرات

✅في حالة عدم استجاب المريض ل maximum bolus does من lV loop diuretics لن يستجيب ل Continuous infusion loop diuretics

The higher efficacy of oral torsemide compared with oral furosemide results from differences in oral bioavailability. There is no such difference in bioavailability when the route of administration is IV

Bolus therapy results in higher initial serum diuretic concentrations and, therefore, higher initial rates of urinary diuretic excretion than a continuous infusion . Thus, in patients who fail to respond to the maximal bolus doses , continuous infusion will be ineffective.

يعني بأن استخدام ادوية ACEI مع dihydropyridine CCB( مثل amlodepin ) افضل من استخدام ACEi مع مدرات thiazide في التقليل من امراض Cardiovascular مثل HF او IHD لكن ع العكس في الفعاليه في التقليل من ارتفاع ضغط الدم اضافه المدرات إلى ACE افضل من dihydropyridine CCB

🛑لو قررت استخدام دواء خافض للضغط هل افضل ان يكون الدواء حبه تحتوي على دوائين single pill combination او اثنين ادوية ؟؟

✅ال ESC 2021 guidelines يجيب بانة نفضل single pill combination لعده اسباب

✅Initial combination therapy (even low-dose combination) is more
effective & faster at lowering BP than monotherapy.
يعني خفض ضغط الدم بيكون اسرع

2.✅ Lower risk of adverse effects.
بتجنب الاعراض الجانبيه للدوائين نتيجة رفع الجرعه

✅Overcome treatment inertia (where patients remain on one drug long term despite inadequate BP control).

يعني اني اوصل إلى target goal بأسرع وقت😁

🛑ماعدا في بعض الحالات مثل

✅ low-risk, grade 1 hypertension (particularly if SBP <150 mmHg),

✅ frail older patients (age >80 years
