متى نستخدم Antibiotic بتحديد (norfoxacin) ك primary prophylaaxis ل SBP في مريض liver cirrhosis ?

متى نقول بأن هناك Diuretic resistance?
بالمختصر
✅Failure of oral furosemide (160 mg twice daily or equivalent) to increase Na+ excretion by at least 90 mmol over 3 days.

✅Fractional sodium excretion <0.2%.

جرعه IV Furosemide في معالجة ADHF تعتمد على جرعه oral التي كان ماشي عليها المريض من قبل

متى نستخدم الانسولين في أمراض T2DM بالنسبه لتراكمي
اذا كان التراكمي فوق 10 ف نفضل إستخدام
basal insulin
وبجرعه
0.1-0.2U/kg

او اذا كان عنده استخدامه اخر مثلا السكر فوق 300 مع اعراض او ماشي ع دوائين او اكثر وما تحسن
طيب في هذه الحاله
نستخدم له انسولين ولكن الجرعه تختلف ع حسب التراكمي

اذا كان نريد نستخدم
Long-acting insulin
اذا كان التراكمي اقل من 8 نعطيه بجرعه
0.1-0.2
واذا كان اكثر من 8 نعطي بجرعة
0.2-0.3

وكيف نعمل معايرة ؟
نعمل كل ثاني او ثالث يوم معايرة
يا اما نرفع الجرعه بمقدار وحدتين من الجرعه الكلية حتى نصل إلى الهدف المطلوب
او نرفع ع حسب السكر الصايم
اذا كلن اكثر من 180
نرفع بمقدار 20% من الجرعة اليوميه
