انت كصيدلاني في حالة نسيان المريض موعد تناول جرعة ادويه مضادات التجلط NoAC مثل Rivaroxaban و Apixaban ،بماذا تنصحة ؟

🛑بنسبة ل ادوية مضادات التلجلط NoAC في حالة نسيان المريض جرعه الدواء ماذا ننصح المريض
✅يتوقف الأمر على كم مره ياخذ في اليوم،اذا كان ياخذ الدواء مرتين في اليوم مثل Apixaban
،بامكانه تناول الجرعه حتى ست ساعات بعد الموعد المحدد ،اما في حالة كان ياخذ الدواء مره واحده في اليوم مثل دواء Rivaroxaban بإمكانه تناول الدواء حتى 12 ساعه بعد موعد الجرعه ،مابعد هذا الوقت لاناخذ الجرعه وننتظر موعد الجرعه الثانيه لناخذها

✅بحيث مرضى الفشل الكلوي CKD في Stage 3 و 4 (GFR 15_60ml/min) ⬅️ معرضين ل الاصابه ب AF بنسبه 15% واحتمال حصول Stroke نتيجة الرجفان الاذيني AF تكون 1.4 أضعاف الأشخاص بدون CKD

✅بينما يرتفع احتماليه الاصابه ب AF في مريض الفشل الكلوي المزمن في Stage 5 إلى 20%
ونسبه حصول stroke تكون من 5 إلى 10 أضعاف

✅After TIA  1 day
🛑 After ischemic stroke according to NIHSS score
✅NIHSS score less than  8 ➡️ reinitiate  oral anticoagulants After 3 days But do CT brain first to exclude hemorrhagic transformation
✅ NIHSS score 8_15 ➡️ reinitiate oral anticoagulants after 6 days  But do CT brain first to exclude hemorrhagic transformation

✅NIHSS score 16 or more ➡️reinitiate oral anticoagulants after 12  days  But do CT brain first to exclude hemorrhagic transformation

-✅Selective B1 blockers (metoprolol or bisoprolol) (class I) -Calcium channel blockers (Verapamil) (class Ila)

-Digoxin (class Ila)

🛑Drugs used for Rhythm control:

✅-Sotalol

Propafenone or flecainide (for structurally normal heart or preexcitation) pregnancy

NB: Amiodarone is contraindicated with pregnancy

يمنع اخذ مضادات التلجلط NoAC مثل Apixaban و Rivaroxaban في النساء الحوامل والمرضعات ،بينما نجد دواء Warfarin آمن في المرضعات وقد يستخدم في الحوامل في حالات معينه .
✅ بنسبه في النساء الحوامل قد يستخدم في المراه مركبه صمام قلب اصطناعي
لو كانت تستخدم دواء Warfarin قبل الحمل بجرعه اقل من 5 ملجم في اليوم ممكن تستمر في أخذه دون توقف حتى الاسبوع ال 36

لكن في حاله كانت تأخذه قبل الحمل بجرعه اكثر من 5 ملجم في اليوم ،ممكن ناخذ الهيبارين حتى الأسبوع 12 من الحمل ثم نرجع ال Warfarin حتى الأسبوع 36 ثم نحوله ع هيبارين حتى قبل ٦ ساعات من الولاده

🛑Indications of human albumin in medicine

✅1- Large volume paracentesis: albumin 20% should be infused after paracentesis of >5L is completed at a dose of 8 grams per liter of removed ascites. Albumin infusion can be considered after paracentesis of lower volumes (less than 5 L) in patients with high-risk kidney.

✅2- Spontaneous bacterial peritonitis albumin 20% should be given within 6 h from the diagnosis at a dose of 1.5 g/kg followed by another dose of 1 g/kg on day 3.

✅3-Hepatorenal syndrome type 1 albumin is given at a dose of 20-40 g daily. It is not recommended in hepatorenal syndrome type II.

✅4- Acute-an-chronic liver failure: there is a week suggestion for the use of albumin over other fluids for resuscitation of these patients in serum albumin level was <3g/L.

✅5- Rexuscitation of patients with septic shock, albumin 4% can be used in these patients when large volume of fluids is required (especially in burn patients and in low serum albumin level) taking in consideration that its benefit is not guaranteed and weighing its potential benefit against its high cost.
