🛑Prevention of Preeclampsia

✅Moderate exercise: ≥140 min/wk➡️
≈40% reduction in preeclampsia

✅aspirin (≥100 mg/day) before 16 wk of gestation ➡️≈60% reduction in preterm preeclampsia

✅dietary calcium intake: if <900 mg/day, initiate calcium supplementation (≥500 mg/day)➡️≈50% reduction
in preeclampsia

✅Timed birth ➡️≈35% reduction
in preeclampsia after 39 wk 0 days

-Consider further diuretic treatment with low dose

انت كصيدلاني في حالة نسيان المريض موعد تناول جرعة ادويه مضادات التجلط NoAC مثل Rivaroxaban و Apixaban ،بماذا تنصحة ؟

🛑بنسبة ل ادوية مضادات التلجلط NoAC في حالة نسيان المريض جرعه الدواء ماذا ننصح المريض
✅يتوقف الأمر على كم مره ياخذ في اليوم،اذا كان ياخذ الدواء مرتين في اليوم مثل Apixaban
،بامكانه تناول الجرعه حتى ست ساعات بعد الموعد المحدد ،اما في حالة كان ياخذ الدواء مره واحده في اليوم مثل دواء Rivaroxaban بإمكانه تناول الدواء حتى 12 ساعه بعد موعد الجرعه ،مابعد هذا الوقت لاناخذ الجرعه وننتظر موعد الجرعه الثانيه لناخذها

✅هناك علاقه بين الفشل الكلوي المزمن CKD والرجفان الاذيني AF
✅بحيث مرضى الفشل الكلوي CKD في Stage 3 و 4 (GFR 15_60ml/min) ⬅️ معرضين ل الاصابه ب AF بنسبه 15% واحتمال حصول Stroke نتيجة الرجفان الاذيني AF تكون 1.4 أضعاف الأشخاص بدون CKD

✅بينما يرتفع احتماليه الاصابه ب AF في مريض الفشل الكلوي المزمن في Stage 5 إلى 20%
ونسبه حصول stroke تكون من 5 إلى 10 أضعاف

✅طبعا في حاله كان هناك Indications ل ادويه statin في مريض الفشل القلبي نستخدم اكيد مثل دخل في MI
✅في مريض الغسيل الكلوي ماينصحش بالبداء بها لكن في حالة كان ماشي عليها قبل الغسيل الكلوي يتستمر عليها بجرعه مضبوطه حسب GFR

When to reinitiate   oral anticoagulants after ischemic stroke
✅After TIA  1 day
🛑 After ischemic stroke according to NIHSS score
✅NIHSS score less than  8 ➡️ reinitiate  oral anticoagulants After 3 days But do CT brain first to exclude hemorrhagic transformation
✅ NIHSS score 8_15 ➡️ reinitiate oral anticoagulants after 6 days  But do CT brain first to exclude hemorrhagic transformation

✅NIHSS score 16 or more ➡️reinitiate oral anticoagulants after 12  days  But do CT brain first to exclude hemorrhagic transformation

✅Rhythm control is preferred over rate control to avoid side effects of the drugs (Rhythm control is usually achieved by DC cardioversion).

-Digoxin (class Ila)

🛑Drugs used for Rhythm control:

✅-Sotalol

Propafenone or flecainide (for structurally normal heart or preexcitation) pregnancy

NB: Amiodarone is contraindicated with pregnancy
