🟥Treatment of Metabolic Acidosis
✅The pH level requiring correction of acidemia is controversial.
✅In patients with DKA, treatment with sodium bicarbonate has not been shown to improve outcomes, even when the pH is ,7.0 🤷‍♂️
.✅In lactic acidosis associated with
sepsis, the parameters for treatment have been a moving target. We do not generally treat lactic acidosis unless the
pH is ,7.1. Unfortunately, treatment has not been shown to improve overall mortality ✅However, in a recent study, treatment with bicarbonate did improve mortality
and decrease the need for RRT in a subset of patients with stage 2 or greater AKI and a median pH 7.15 There-fore, in this population, the use of bicarbonate is recommended.
