Standard: is a substance of known purity which is used as the basis for a comparison of measurements in the determination of this substance in a specimen.

🔴من إحدى المتطلبات الاساسية، لإعطاء نتائج صحيحة ومضبوطة في المختبرات العلمية بشكل عام والطبية السريرية بشكل خاص، هي استخدام محاليل عيارية معلومة التراكيز، ذات خصائص مناسبة من حيث النقاوة والاستقرارية، وغيرها من الخصائص التي تشترطها جهات متخصصة، لقبوله كمحلول عياري او ما نعرفه بالـ: استاندارد.

🔴يعرف الاستاندارد بانه مادة نقية جدا، لا تحتوي على أي مواد اخرى مهما كانت تراكيزها ضئيلة.. لا مواد حافظة.. ولا ماء.. ولا اي مواد اخرى، وان لزم الامر للاضافة، كما في بعض انواع الاستندرات(سنذكرها هنا لاحقا)؛ فلابد ان تكون بالكميات القليلة الضرورية فقط التي لا تغيّر من خصائص هذه المادة بعد الاضافة ، وإلا فلا يعتبر مادة عيارية او قياسية.
ويحتوي الاستاندارد على مادة بعينها تكون معلومة التركيز تُستخدم لايجاد تركيز مادة اخرى مجهولة التركيز بالمقارنة وبإستخدام قوانين معروفة، كقانون بير لامبرت او غيره من القوانين ذات الصلة.

تُقسّم الاستندارات، بحسب تصنيف الاتحاد الدولي للكيمياء البحتة والتطبيقية IUPAC، واعتمادا على درجة نقاوتها الى 5 درجات:
Grades: A,B,C,D and E


1- Atomic-weight Standard (Grade A)
2- Ultimate Standard (Grade B)

♦️عبارة عن استندارات ذات نقاوة عالية جدا، تصل نسبة الخطأ في نقاوتها الى 0.0% ( صفر% )، ، فهي استندرات على اعلى مستوى من النقاوة لا تشوبها شائبة، فلا تحتوي على أي مواد جانبية اضافية اخرى البتّة... تراكيزها معروفة وموزونة بدقة متناهية، لا نقول بالـ مل جرام او الميكرو او النانو جرام ولا حتى بالبيكو او الفيمتو وووالخ، وانما بوحدات وزنية اصغر من ذلك بكثير تعرف بالـ "يوكتو جرام" (10¯²⁴) اوما يعرفه الغالبية "بالدالتون"، وتكون على شكل بودرة مجففة. يمكن انتاج مثل هذه الاستندرات مع المواد الغير عضوية في بعض فروع الكيمياء كالكيمياء التحليلية ، ويصعب انتاج مثيلاً لنقاوتها في فروع اخرى كالكيمياء السريرية، لاسباب سنعرفها اثناء تقدمنا بالشرح اكثر..

♦️ونظراً لنقاوتها العالية؛ يمكن معرفة تراكيز مثل هذه الانواع من الاستندارت حتى من دون تحليل كيميائي و حتى من دون استخدام اجهزة مقياس الطيف:
فاذا ما افترضنا ان وزن مادة ما، معطاة لدينا داخل انبولة، كان 1 جرام، وان وزنها الجزيئي 25 دالتون.
فالتركيز يكون (عند اذابته في لتر من المذيب المناسب) كالتالي:
25/1 ← 0.04 مول/لتر←0.1 جرام/دل ← 100 مل جرام/دل.

♦️الاستندرات ذات الدرجات A,B بالاضافة الى ذات الدرجة C ) primary Standard ) لا تنتجها الا مختبرات معروفة لا يتعدى عددها عدد اصابع اليد الواحدة في العالم اجمع كالـ IUPAC (المصنّف الرئيس للاستندرات وواضع شروط درجات النقاوة). بالاضافة الى المكتب الوطني للمعايير في الولايات المتحدة NBS.
بعد انتاجها يتم توزيعها على شركات انتاج المحاليل او على المختبرات المرجعية العالمية؛ ليتم تحضير استندرات اخرى منها ذات درجات نقاوة اقل نوعاً ما كـ استندرات الدرجات D and E. ولا نُقلل -نحن هنا- من درجة نقاوة مثل هذه الاستندرات، عند وصفنا لها بأنها "اقل نقاوة"، فهي لا تزال محتفظة بشروط نقاوة تصنيف الـ IUPAC( راجع نسبة الخطأ في التحضير، تجدها غير ذات اهمية خاصة فيما يخص الكيمياء السريرية).. فمثلا اذا تم تحضيراستاندارد من الجلكوز ذو تركيز 100 مل جرام لكل دل، فيجب ان لا يتعدى نسبة الخطأ في تحضيره عن 0.05% ( أي يكون 99.95 مل جرام لكل دل). فلا فرق هناك يتطلب منك مجهوداً.. ولو مجرد تحريك النظر.. الى ماهية الرقم بعد الفاصلة العشرية .. بين التركيزين 100 وَ 99.95 .
كل ما كانت النقاوة عالية؛ كلما كان الثمن اعلى.

🔰في فروع من الكيمياء العامة، كالكيمياء الغير عضوية، يمكن انتاج استندرات ذات نقاوة من الدرجات الاولى( نسبة الخطأ في التحضير تصل الى صفر%)، بينما توجد صعوبة في فروع اخرى من الكيمياء كالكيمياء السريرية مثلاً، في انتاج استندرات ذات درجات نقاوة مماثلة؛ نظراً للتركيب المعقّد للمواد العضوية، كما ان طرق تنقية بعضها لم يتم تعريفها بشكل كامل وواضح بعد. بالاضافة الى ان التجفيف لبعض المواد العضوية يفقدها خواصها… وعلى الرغم من امكانية تجفيف بعضها- كالبليريوبين مثلا- إلا انه سرعان ما يفقد خواصه كمحلول عياري بعد اذابته في الماء.
تحتوي هذه الانواع من الاستندارات على مواد اخرى اضافية ضرورية الى جانب المادة المعنية كالماء مثلاً، حتى وان كان الاستاندارد على شكل بودرة. فقد تأتي مثل هذه الاستندرات سائلة (محضّرة جاهزة)، او بإذابتها في المذيب المناسب ان كانت على شكل بودة..

وبعكس الاستندرات A,B,C، لايمكن هنا معرفة تركيز الاستندارد بوزنه قبل تحضيره، حتى وان كان على شكل بودرة، إن لم يكن مكتوبا عليه.. الا بالتحليل الكيميائي بمقارنته باستاندارد اخر معلوم التركيز.
فلو فرضنا ان وزنه قبل التحضير كان 1 جرام، فنحن هنا عرفنا فقط ان الوزن الكلي 1 جرام، ولكننا لم نعرف كم يساوي وزن المادة المعنية من هذا الجرام، وكم وزن المادة او المواد الاخري المتواجدة معه.. بينما الجرام الواحد للمادة المعنية، في الاستاندارات المجففة ذات الدرجات الاولى، يساوي واحد جرام منها؛ لانه لاتوجد في العلبة أي مواد اخري غيرها.

♦️Standard Reference Materials (SRM):

في السنوات الماضية الماضية القريبة ، ونظرا للصعوبة في تطبيق شروط IUPAC لانتاج استندرات لبعض المواد العضوية، جاء المكتب الامريكي للمعاييرNBS بشروط اقل صرامة فيما يخص النقاوة الواجب توافرها لإنتاج محاليل قياسية لمثل هذه المواد اسمتها SRM، ويمكن الحصول عليها منه...
فقط المواد الغيرعضوية، والتي ينتجها هذا المكتب، تخضع لشروط IUPAC.

🌺 Standard and caliberator :

🔰اغلب ما يصل الينا من محاليل عيارية هي ذات درجات D&E ( راجع انواع الاستندارات الاخرى في الاعلى). نستخدم هذه المحاليل العيارية إما:
1⃣لإيجاد تركيز ماة اخرى مجهولة التركيز بمقارنة تركيز الاستاندارد اليها، ونسميه هنا standard او
2⃣لمعايرة جهاز ما، لمعرفة ما اذا كان كل شئ يسير على ما يرام ونسميه هنا calibrator.

☜شركات انتاج المحاليل، خاصة ذات الانظمة المغلقة، تأتي بمحاليل فحوصات ومعها محاليل عيارية(كالبريتر) خاصّة بها.. وكليهما ذات اكواد شفرية barcode تحتوي على كل المعلومات المتعلقة بهم..
الاكواد الشفرية لمحاليل الفحوصات عادة تحتوي على معلومات كمعلومات معايرة محلول الفحص، والفاكتر ومعلومات اخرى كثيرة، تنتقل هذه المعلومات الى الجهاز عند عمل مسح لهذا الكود الشفري بواسطة الجهاز للمرة الاولى، ولكن ومع هذا يطلب منك الجهاز ادخال الكالبريتر لعمل معايرة!! ليس لايجاد منحنى واستخراج فاكتر الحساب لمحلول الفحص، فالجهاز قد حصل على مثل هذه المعلومات مسبقاً من شفرته التي زودته بها الشركة المصنّعة، وانما يطلب الكالبريتر فقط للتأكيد من ان كل الخطوات المطلوبة لاجراء الفحص المعني صحيحة.
☜فالمحلول العياري، الذي يأتي مع محلول الفحص، ايضا يحتوي على شفرة كودية بها معلومات من ضمنها تركيزه، فإذا ما عملنا معايرة، بوضعه على الجهاز، وكان التركيز الناتج هو نفس التركيز الذي حصل عليه الجهاز عند مسحه لشفرته؛ فإن الجهاز سيقبل محلول الفحص ويعطيك اشارة تدل على ان كل شي على ما يرام,,, اما اذا لم تكن نتيجة فحص تركيز الكالبريتر متطابقة مع ما حصل عليه من شفرته؛ سيرفض الجهاز المحلول المعني، وعليك ان تجد السبب.... قد يكون محلول الفحص غير صالح وعليك استبداله، او ان المحلول العياري هو نفسه غير صالح او ان هناك اسباب اخرى كعدم استقرار التيار الكهربي او ان هناك حاجة لصيانة الجهاز للتأكد مثلا من انسدادات او اوساخ او التأكد من لمبة او خلية القياس ووووالخ اسباب كثيرة قد تجعل من محلول الفحص مرفوضاً.
👌بالمختصر فأن الاستاندارد يستخدم: لأنتاج استندارد اخر اقل نقاوة نوعاً ما، او لإيجاد تركيز مادة اخرى مجهولة، بينما الكالبريتر يستخدم لمعرفة ما اذا كان جهاز ما او اداة ما يعمل او تعمل بشكل صحيح ام محتاج لضبط..
فمثلا اذا كان لديك قطعة حديد وزها واحد كيلو جرام.. قطعة مثل تلك المستخدمة في الميزان الكفّي... فممكن استخدمها كـ استاندارد لايجاد وزن سلعة ما، بوضع قطعة الحديد ذات الواحد كيلو جرام في احدى كفتي الميزان ووضع السلعة في الكفة الاخرى ... هل هما متساويان ام ان السلعة وزناها اعلى او اقل.. وممكن استخدم قطعة الحديد هذه كـ كالبريتر لمعايرة ميزان رقمي مثلاً . لديك ميزان رقمي وتريد معرفة هل يعمل بشكل صحيح ام لا، فتضع قطعة الحديد ذات الواحد كيلوجرام عليه، وتنظر كم وزنها عليه فتعرف عندها: هل هذا الميزان يعمل بطريقة صحيحة ام يحتاج لضبط.

🔯Pituitary Tests

Introduction: -
Pituitary disease, like other endocrine disease, is fairly common, and is generally treatable if (and only if!) you discover it on time.

The Adenohypophysis ("anterior pituitary")
⇶produces ACTH, TSH, FSH, LH, GH, and prolactin.

💢 If the input from the hypothalamus is interrupted, the adenohypophysis makes more prolactin and less of everything else.

🔺Hyperpituitarism: -
is defined as too much of one (or maybe two) of the hormones from the adenohypophysis.

💢This may be due either to autonomous overproduction
(i.e., from a primary adenoma here, true adenocarcinoma of the adenohypophysis is very rare from excess production of hypophyseal stimulating factors or underproduction of inhibiting factors, or loss of inhibition following destruction of other endocrine glands. ☞Some genetic syndromes are also known.

🔻Hypopituitarism
("Simmonds's disease"): -
is defined to mean loss of one or more (often all) of the hormones from the adenohypophysis.
●Patient needs to lose about 75% of the mass of the adenohypophysis before any clinical changes occur.

The illness is underdiagnosed and deadly.

🔸For these, a random sample of blood isn't much use in determining whether your patient is making enough of a particular hormone.

🔸To confirm endocrine insufficiency, you'll usually perform a Stimulation Test.

🔸 In other words, you'll do something that should make the patient produce the hormone in question.

🔸 If enough of the hormone does not appear in the blood, you have confirmed that disease is present.

♐Low dose dexamethasone suppression test:-
2 mg of dexamethasone, a synthetic glucocorticoid, given at midnight, should drop one's ACTH and cortisol to near-zero by 8 AM.

♐High dose dexamethasone suppression test: 8 gm of dexamethasone is usually enough to suppress an ACTH-producing pituitary adenoma and the cortisol production that results from it.

🔻Low ↓ ACTH

Screening tests based on spot-drawn hormonal levels have some use, but results are often indeterminate.

👉CRH test: Give corticotropin releasing hormone by vein. Look for an appropriate increase in ACTH and cortisol.

👉Insulin tolerance test: Give insulin to drop the blood glucose below 40 mg%.
Look for an appropriate increase in serum cortisol.

👉 ACTH stimulation test: Give ACTH and see if you get a cortisol and aldosterone response.
Of course this is really testing the adrenal gland's ability to respond, but if the pituitary hasn't been making ACTH, the adrenal will be atrophic.

💢You can repeat the test over the next few days, and if the response is better each time, the problem is in the pituitary; if not, the problem is in the adrenal.

In suspected "syndrome of inappropriate ADH secretion", the basic findings (hyponatremia not otherwise explainable, inappropriately high urine sodium levels and concentration, no edema, good response to water restriction) are a lot more help than any additional labs.

💢NB! Don't order a plasma ADH if you're looking for SIADH

You'll suspect ADH deficiency when your patient is putting out a lot of urine that is hypotonic (SG <1.010, concentration <300 milliosmoles/L).

☯Restrict water for a while, perhaps overnight and/or until the patient is uncomfortable. If the urine does not rise above 1.010 SG / 300 mOSM/L, you have diabetes insipidus. To see whether ADH deficiency is the cause (rather than a kidney tubule problem), give an injection of ADH (as desmopressin).

➿Experienced physicians may administer hypertonic saline to get the test over with, and/or assay plasma ADH before and during the procedure.

Most patients with pituitary adenomas producing gonadotropins make alpha and beta chains rather than intact hormones.

Baseline levels are okay, and you can also order estrogen and testosterone levels.

♐ Check for normals for age and sex; remember that if a post-menopausal woman's FSH and LH levels aren't high, something may be wrong.

💢GnRH test: If you must do a stimulation test, this is the way to see whether FSH and LH can be raised.

In growth hormone deficiency, it will be low.

Nowadays, especially for "adult growth hormone deficiency", some clinitions prefer a stimulation test.

Various suppression tests have been proposed, but a series of spot-checks in the morning seems to be the usual.
