🔰Nephrotic syndrome
====================
🔰Nephrotic syndrome caused by increased permeability of serum protein through the damaged glomerular basement membrane(GBM) in the nephron.

🔰Mechanism of nephrotic syndrom may be related to :
-------------------------------
♦️Molecular barrier injury: holes on GBM become larger.
♦️Charge barrier injury: loss of negative charge within the GBM.

🔰Findings
----------------
1⃣Proteinuria:as a result of an increased in glomerular capillary walls permeability whitch allow proteins to leak into the urine.

2⃣Hpoproteinemia:as a result of protein loss in urine.

3⃣Oedema: as a result of a decreased in oncotic pressure whitch allows fluid to move out of the intravascular compartment and into the surrounding tissues causing oedema.

4⃣Hyperlipidemia: as a result of decreased plasma oncotic pressure, which stimulates hepatic very low-density lipoprotein synthesis and secretion.

5⃣Hypercoagulability: as a result of renal losses of proteins C and S and antithrombin III, as well as elevated serum fibrinogen and lipid levels.

🔰Loss of other plasma proteins besides albumin in nephrotic syndrome may present as any of the following:
6⃣A defect in bacterial opsonization and thus increased susceptibility to infections (eg, as a result of loss of IgG).
7⃣Vitamin D defciency state and secondary hyperparathyroidism (eg, resulting from loss of vitamin D–binding proteins).
8⃣Altered thyroid function tests without any true thyroid abnormality (resulting from reduced levels of thyroxine-binding globulin).

♦️في الحالات الطبيعية ترشح الكُبيبة الكلوية جزيئات وتمنع ترشيح جزيئات اخرى. و يُعزى ذلك لإحتواء الكبيبة على المسامات الدقيقة ، وعلى الشحنات السالبة . وفيما يخص البروتينات ، فإن الكُبيبة الكلوية تسمح بمرور البروتينات ذات الاوزان الجزيئية الاقل وزناً من الالبيومين - اي مَن كانت اقطارها اقل قُطْراً من قُطْر مسامات الكبيبة - كما تلعب الشحنات السالبة ايضا دورا مهما في توليد عملية التنافر. وعلى الرغم من ان الالبيومين ، يعتبر اكبر قطرآ من قطر مسامات الكبيبة نوعا ما ، ويحمل بنفس الوقت شحنة سالبة مشابهة لشحنة الكُبيبة الكلوية مما يجعله يتنافر ، إلا انّ كمية منه تمرّ عبر مسامات الكُبيْبة نتيجة لفرق الضغط الحاصل داخل الشعيرات الدموية للكبيبة وخارجها في تجويف كبسولة بومان. فيُرشّح ما تركيزه حوالي 2000 ㎎ يومياً ، ويُعاد امتصاص اكثر من 97% من هذه الكمية المفلترة عبر الانيبيبات القريبة والبعيدة والملتوية في الكلية وتُحلّل في lysosome خلايا هذه الانيبيبات الى احماض امينية ويتم اعادتها الى الدم للاستفادة منها. وتخرج النسبة المتبقية ، والتي لم يُعد امتصاصها من الالبيومين ، مع البول وهي لا تتجاوز 30 ملج / 24 ساعة، وهي تعتبر نسبة طبيعية.
♦️في حالات مرضية كما يحدث في nephrotic syndrome ، وعلى حسب المسبّب ، فإن مسامات الكبيبة الدقيقة تتوسع نوعا ما ، او تفقد الكبيبة شحناتها السالبة ، و قد يحدث الامران معاً ( اسباب كثيرة لهذه الحالة ) ، فنجد تركيز الالبيومين الخارج مع البول يتجاوز الـ 3000 ملج في اليوم ( الطبيعي 30 ملج).

♦Tip:
لسهولة تذكّر ما يترتب على هذه الحالة من نتائج واعراض ، ولتجنب خلط الاوراق مع الحالة الاخرى nephritic syndrome ، والمشابهة لها إلا حرفا واحدا ( O مقابل I ). تذكر انّ كلمة nephrotic تحتوي على الحرف “O” و انّ كلمة protein
ايضا تحتوي على حرف "O"
فإذا ما تذكرت البروتين ، وزيادة خروجه في البول (Proteinuria) ، تذكر ان هذه الزيادة الخارجه منه مع البول ، ستُقلّل من تركيزه في الدم (Hypoproteinemia) ، وانّ قلة التركيز له بالدم ستخفض من الضغط الانكوتي ، فيتسرب الماء الى خارج الدورة الدموية - الى ما بين الانسجة - مسبباٌ التورم (edema) وكإستجابة و في محاولة لإحتواء الامر ، يزيد الكبد من تصنيعه للبروتينات ومن ضمنها الليبوبروتين والتي تؤدي الى ارتفاع دهون الدم (Hyperlipidemia)
Proteinuria→ Hypoproteinemia→Edema

☸ Nephritic syndrome :

♦️Nephritic syndrome is a group of symptoms that cause swelling and inflammation of the glomeruli in the kidney.

♦️Causes :
========
Acute nephritic syndrome is often caused by an immune response triggered by an infection or other disease.

♦️Diagnosis:
=========
1⃣Haematuria:
Haematuria occurs due to podocytes developing large pores which allows blood and protein to escape into the urine.
2⃣Red cell casts
distinguishing feature and indicate glomerular damage.
3⃣Proteinuria:(small amount).
4⃣Hypertension:
5⃣Low urine volume _<300ml/day_: Due to reduced renal function.
♦️الـ nephritic تحتوي على الحرف "I" والـ cytes (اشارة للخلايا الحمراء الخارجة مع البول تحتوي حرفياً على الحرف "y" ، ولكنه صوتياً كحرف "I"… هذا قد يساعد عند التذكر للتفريق بين هذه الحالة ، والحالة الاخرى السابق شرحها nephrotic syndrome ، والتي تحتوي على الحرف "O" والبروتين الزائد في البول والمفرّق لها يحتوي على الحرف "O".
