كانت PROM بالشهر الثامن قالت نزل عليها الماء ومافيش عندها اي حاجة ثاني
دخلت لعند ال report ناقشتها مع الدكتورة حنان وسألتني ايش تعريفه و ال risk factors وال differential diagnosis و على ال chorioamnioitis وكيف تشخصيه ايش الفحوصات

Genera examination
قالت اسويه كامل بدأت المقدمة كامل وسألت المريضة وين هي وايش اليوم وعلقت على BMI وشكل المريضة وال position والكانيولا والكاثيتر
وبدأت من اليد وصلت للكتف قلت لها على ال pressure قال افحصي وضاع الوقت كله وانا عنده ماعد لحقت الا اشوف ال face بس

Local
قالت افحص inspection of abdomen لمريضة بعد عملية
قلته كامل وماسألت شي وبقي وقت فاضي

Obs examination
الدكتورة جيهان حبوبة سويت ال leopold maneuver وسألت ايش خصائص ال head وال breech بس

Investigation
كان فحص ال rubella والاسئلة كانت ايش ال MOT وايش ال complication وال prevention

Partogram
كان normal labor وسألتني اذا جت بال 2nd stage ايش تسوي وقلت لها العملية كامل اللي بالملخص وسألتني على ال forceps وال vacum متى استخدمهم وايش الشروط

Drugs and contraceptives
جابت لي COC وسألت ايش ال MOA و كيف تستخدمه وايش الحالات اللي ممكن تحمل مع الحبوب تقولي اذا نست حبه او اذا استخدمت مضاد حيوي او antiepileptic drugs بهذه الحالات المفروض تشرب حبتين ..وسألت اذا نست حبه ايش تسوي تقولي تشربها متى ماذكرت وسألتني اذا واحد زوجها بيجي من السفر وهي عاشر يوم الدورة وتشتي تشرب هذه الحبوب ايش تقولي لها.. الحل انه تبدا تشرب عادي بس avoid sexual intercourse for 7days
وجابت لي methargen قالت متى استخدمه وايش ال SI وال contraindication

Instrument
كان obs forceps والاسئلة متى نستخدمه وايش ال complications

Emergency
حالة واااضحة eclampsia وسألتني كيف بسوي ال mangment تقولي اول شي call for help وبعدين تفصلي ABCDEF وسألت على العلاجات اللي بتديهم وال mag sul. ايش ال SI و contraindications وال antidote واخر شي تقولي termination of pregnancy regardless the gestational age

ومحطة skills
كانت episiotomy كيف تقطعي ومتى ونوع الخيط وكيف تخيطي وايش تنصحيها بعدها انها تخلي المنطقة ناشفة ونظيفة وتاكل خضروات وسوائل عشان مايحصل امساك

- اول محطة كانت drugs and contraceptive: وكان تمام سألتني على الهيبارين ايش يسوي ومتى نستخدمه وايش الside effect حقه وكمان ادت لي implanon ونفس الاسئله ....

- بعدين محطة البارتوقرام : جابت لي واحدfetal distress وشافتني كيف احلله وبعدا رجع قالت لي لو كان كذا كذا ايش تسوي بس وشروطventose...

- محطة ال instruments: جابت لي sims speculum : وسالتني uses و advantage وَdisadvantage


- محطة الskills : active manegment of third stage والخطوات كامل لازم تشرحيها
- محطة الlocal : قالت لي اعملي inspection of abdomen بس نفس الي نعرفه

- بعدا محطة الgeneral : قالت اسوي فحص عام ونفس الي اخذناه مافيش فرق


- محطة ال obs Ex: قالت اسوي ال lepold manoeuvre بس.

- اخر محطة كانت emergency : وكانت حالة post partum. haemorrhage due to uterine atony ايش عتفعلي لها

وجابت ال COPPER "IUCD
متى نعمله وال S. E تبعه. وال Advantages وال disadvantages

- بعدين محطة البارتوقرام : واحده امروها طيبه وكل شي تمام بس كان عندها الضغط مرتفع وال. Proteinuria +3 كانت PET وكيف انولدها كانت بال Forceps وسالت عن شروط استخدامه.

- محطة ال instruments: جابت لي Hegar dilator
وسالتني uses و وَdisadvantage
وSE


- محطة الskills : قالت واحده قدي fully dilated ايش اعمل واذكر لها خطوات ال 2nd stage كامل
- محطة الlocal : قالت لي اعملي inspection of abdomen كانت المريضه sever pain

- محطة الgeneral : قالت اسوي فحص عام ونذكر كل شي نعلق على ال cannula وايش لونها وفي اي يدي مريضتي كان معاها 2 cannula
وتعلقي على Catheter نعلق على BMI وكل هذه الحاجات


- محطة ال obs Ex: قالت اسوي ال lepold manoeuvre وسألت هل هوه cephalic والا breech


- محطة الinvestigation :
كانت الصبغه HSG لايش تستخدم وايش انشوف وكيف شكل ال uterus وال SE حقه
- محطة الهستوري : vaginal bleeding من يوم لوحده حامل في السابع حقل لها Trauma


الهستوري : مع الدكتورة حنان قدمت الهيستوري وبعدا وسالت على الrisk factor وايش الD/D
- محطةال emergency : وكانت حالة PID

أول محطة أنا دخلتها كانت emergency
دخلت كان في ورقة قرأة منها الحاله
Patient complaining of acute abdomen with high grade fever and has tender adnexal mass after u.currtage
المطلوب
1 _ diagnosis (tubu-ovarian abscess ) of PID

وطبعا تسأل عن كل شي في الحاله من فحوصات وتشخيص ومعالجه ...ولو نسيتي شي تذكرك

ثانيا دخلت محطه partograph
كانت حالتي
Sever PET with progress of labor
Management:
Shorts second stage by forceps delivery +Hydralazin +Mg sul as prophylaxis

ثالثا دخلت محطه
General examination
وطبعا ع مريضه كانت
وتفعلي كل الي درسناه ب الجينيرال
وطبعا كانت تقلك اشرحي لي الي تفعلي يعني افحص واتكلم ولا تنسو تشيكو على الأدوات سواء كانت كانيولا أو كاثيتار و BMI

و كيف نعرف Pt عنده allergy

بالنسبة لاختباري كان :
Long case: كان مع الدكتور أمل

Esophageal constriction طبعا أخذت الهستوري وحللت
C/C المريضة عمرها 7 أشهر أجت ب dysphagia ..
كتبت الهستوري كامل بكل محتوياته ولا عقبت عليا ولا ناقشتني لانه مكتمل وطبعا عملت general appearance و anthropometrics measurement و vital signs ....... وعملت فحص للجهاز الذي هو affected (only GIT) وبعدين قالت لي examine respiratory system وطلعت لها ال findings .
طبعا الطفلة كان عندها signs of respiratory distress والدكتورة قالت لو الطفلة جت لك الطوارئ أيش عتدي لها ؟ نمشي مع نفس خطوات approach to shortness of breath اللي هي stabilization(ABCD) ،، Supportive وتفصلوا فيهن وكملت معاها...
Short case:
كانت مع الدكتور عبد الوهاب الحيفي وطلب مني اعمل cns و cardiac و respiratory ..
وقلي اطلع ال findings الذي هي علامات down الواضحة على الطفل ،، بعدين سألني أيش ال congenital anomalies of heart وايش ال most common منها .....
وقال لي الطفل جاء لعندك للطوارئ ايش تعمله ، طبعا الطفل كان عنده sings of respiratory distress ...
وبعدين قال قبلما يخرج ايش تنصحة يفعل ، قلت له يعمل Echo عشان نسوف ال heart ...وكملنا

- محطة مناقشة الHx : المطلوب differential diagnosis, risk factor, investigation and treatment


-محطة الemergency: كانت الحالة ectopic pregnancy


- بعدمحطة الskills : كان vaginal swab



- محطة ال local examination : قالت اسوي abdominal inspection


- محطة ال obs Ex: قالت اسوي ال lepold manoeuvre والfundal level.


- محطة الgeneral examination:
كامل زي ما اخذنا قبل


محطة الpartogram: كانت الحالة arrest of head engagement and management


وآخر محطة اللي هي الdrug and contraceptive:
