هذا المخطط يشرح خطة علاج حالة الـ GAD

1. Diagnosis of Generalized anxiety disorder GAD
تشخيص الحالة
ذكرت الكم بالتشخيص انه الاعراض لازم تستمر عالاقل 6 اشهر + عالاقل 3 من الـ Physical symptoms بالاضافة انه ال symptoms كون تأثر على حياة وعمل المريض functioning impairment



اوك هسه فعلا شخصنا المريض عنده GAD
انطي علاج مباشرة؟

لا طبعا ، هنا لازم الطبيب يسوي

2. Evaluation
لازم يتأكد اذا المريض عنده مشاكل ثانية تأثر على القلق لو لا

مثلا Comorbid conditions اذا عنده مشاكل جسدية او امراض نفسية ثانية تصاحب حالة الـ GAD مثلا عنده depression او عنده bipolar disorder او نوع ثاني من ال anxiety

بعد الـ evaluation، هنا تجي خطوة الـ

العلاج يا اما يكون
4. psychotherapy
علاج نفسي مثلا:
Psychosocial Therapy PST
ممكن يكون فعال لوحده أو مع
الادوية

او Medical Therapy العلاج بالادوية

5. SSRI/SNRI: 4-6 week evaluation with adequate dosing
الخط العلاجي الاول انه ننطي antidepressants بأستخدام دواء من ادوية SSRI أو SNRI،
بعد 4 الى 6 اسابيع نقيم استجابة المريض للدواء

لفترة 4 الی 6 اسابيع

الخطوة الي بعدها

6. Assessment for initial response

راح المريض للطبيب
رح يسأل عن التزامه وتجربة العلاج، فاذا :

❌ Inadequate trial
يعني تجربة المريض جانت غير كافية
يوم ياخذ دواء ويوم لا يعني ما ملتزم nonadherence فهنا شوفوا السهم راجع من جديد ع الادوية الي برقم 5 هاي الخطوة يسموها:
Observed Therapy
العلاج تحت المراقبة، يعني إجبار المريض على الالتزام بالدواء ، المهم يرجع يعيد العلاج من البداية



✅ adequate trial and good response

يعني اذا تجربة المريض للعلاج جانت كافية
الجرعة صحيحة وكاملة و ملتزم بالعلاج ١٠٠٪ طوال هذه الفترة
وجانت الاستجابة جيدة يعني المريض متحسن على هذه الادوية

الـ7aـقرار هو الاستمرار على هذه الادوية لمدة سنة واحدة عالاقل

🔃 Adequate trial
يعني المريض ملتزم ١٠٠٪ بس المشكلة انه الاستجابة جانت جزئية او اصلا نهائيا ما استجاب

🧩 فاذا partial response الاستجابة الجزئية
8. Assessment for partial response
نقيمها نشوف شنو ال symptom المتبقية:
🔹 If insomnia
اذا عنده ارق فهنا نحتاج ننطي ادوية تساعد المريض يهدأ وينام مثلا
9. Hypnotics: non BZD GABAergic Hypnotic, BDZ or trazodone, mirtazapine, or antihistamine

🔹 If full symptom persistence

اذا اغلب الاعراض باقية فهنا عدنه طريقتين:
10. Augmentation
تقوية العلاجات الأولى الي انطيناهن الي هنه ال antidepressants ب ادوية ثانية مثلا:
Atypical Antipsychotic
BZD, antihistamine, buspirone
او من خلال اضافة العلاج النفسي PST

11. Switching to another AD
نغير الدواء الي جنت انطيه بالبداية اذا جنت انطي ssri احوله لل snri و العكس صحيح


🧩 واذا كانت الحالة non response رغم الالتزام بس ماكو استجابة نهائيا
نسوي الخطوة رقم 11 نفسها 👆


وراها اسوي تقييم للاستجابة

اما اذا جان partial or no response

13. Evaluate for significant Comorbidity
نرجع نتأكد اذا اكو Comorbid condition او لا



🔴 اذا اي موجودة حالات مصاحبة للGAD ف نشوف شنو الحالة :
♓ IF Depression
14. ننطي جرعة كافية من ال Antidepressants او نضيف وياه ادوية ثانية augmentation مثلا buspirone, bupropion, Atypical antipsychotic او ECT

♓ IF bipolar disorder
15. ننطي mood stabilizer مثلا lithium او anticonvulsant او atypical antipsychotic

♓ IF other anxiety disorder

16. يعني اذا اكو حالة anxiety مع الـ GAD

A. GAD and Panic disorder
نضيف TCA او SSRI/SNRI او BZD

B. GAD and social anxiety disorder SAD
ننطي BZD, SRI, AAP, Pregabalin, levtriacetam
C. GAD and Obsessive Compulsive disorder ‌OCD
وسواس قهري
نضيف SSRI او Cloimpramine

D. GAD and PTSD
نضيف SSRI, SNRI, APP or prazosin

🔴 اذا ماكو
17. switch to another combination
نحول العلاج الی combination دمج أدوية من فئات مختلفة
او نضيف وي العلاجات الأولية دواء ثالث او نضيف العلاج النفسي

وراها ارجع اسوي تقييم
18. Assessment for response
نشوف صارت استجابة لو لا

اذا صارت استجابة وتحسنت ال symptoms

الـ7cـقرار هو الاستمرار على هذه الادوية لمدة سنة واحدة عالاقل

اذا ماكو استجابة او جانت الاستجابة جزئية نرجع نعيد التشخيص من البداية
