▪️Disappearing pulse:
When collateral circulation is very good, peripheral pulses may be normal. However, when the patient is asked to exercise, the pulse may disappear.

✨Exercise produces vasodilatation below the obstruction and arterial inflow cannot keep pace with increasing vascular space, pressure falls and the pulse disappears

♨️Fever with chills or with rigors 🤔?


♦️Chills👉🏻the feeling of coldness.
-can come without fever (feeling cold after being in a cold environment)(*1)

-and unlike Rigors chill is a common association with most fevers.(*1)

🖇It is important to know that, Fever with rigors is of a poor diagnostic value " non-specific as both are commonly reported by patients with many diseases"(*2)


♦️Rigors👇🏻

▪️Definition:
-Rigors are generalized, uncontrollable episodes of vigorous bady shaking lasting a few minutes , often with ‘chattering’ teeth.(*2)
Rigors can be also defined as profound chills🌚(*1)

▪️Pathophysiology:
-Rigors are associated with rapid temperature rises and may be caused by cytokines and acute-phase proteins resetting the hypothalamic temperature set point.(*2)

🖱However many authors have been illustrated the combination of fever & rigors as the following(*3) 👇🏻

🔥Fever + rigors = infections or reactions.(*3)
📌Infections as :
🔹.Amebic hepatitis,
🔹.Acute pyelonephritis
🔹.Abscess
🔹.Brucellosis
🔹.Pyaemia
🔹.Septicemia
🔹.Malaria

📌Reactions as:(*3)
🔹.Pyogenic reaction
🔹. Hemolytic crisis
🔹. Charcot's fever

‼️Fever+Rigors can be misinterpreted as febrile seizures but the retention of consciousness & associated pyrexia suggest rigors.(*2)
