A 26-year-old woman presents to the emergency department with numbness and weakness in both legs. She had difficulty standing and walking. She also reports urinary urgency and two episodes of involuntary urine leakage. She has no history of back pain or trauma. Four years ago, she was diagnosed with type 1 diabetes mellitus, and her blood sugar is managed with a continuous subcutaneous insulin infusion pump. One year ago, she was evaluated for right eye pain and blurred vision, but these symptoms resolved spontaneously within a week. Her temperature is 36.9°C (98.4°F), blood pressure is 134/78 mmHg, and pulse is 76/min. Visual acuity and pupillary reflexes are normal. Muscle strength in the right and left lower extremities is 3/5 and 4/5, respectively. Patellar reflexes are 3+ bilaterally, and plantar reflexes are upgoing. Vibration, light touch, and pain sensation are decreased in both legs. Her fingerstick glucose level is 144 mg/dL.

A 9-year- old boy is brought to the emergency department following a bicycling accident. The patient was riding downhill when he hit a rock and lost controle. He was thrown off his bicycle and hit the handlebars befor tumbling onto the pavement. The patient was not wearing helmet but didn’t lose consciousness. BP is 118/80 mmhg and pulse is 140/min. examination shows ecchymosis over the right superior orbit with underlying tenderness and few scalp abrasions. Visual acuity is 20/20 in the left eye. Using the right eye, the patient is unable to read the largest letter on the visual acuity chart and is only able to perceive light. There is a relative afferent pupillary defect in the right eye. Extraocular movements are normal, and the red reflex is symmetric bilaterally. Fluorescein dye testing is unremarkable. Which of the following is the most likely diagnosis in this patient

34-year-old man is brought to a rural emergency department after being rescued from a burning building where he was trapped for a prolonged period.  The patient reports a headache and dizziness.  Temperature is 36.9 C (98.6 F), blood pressure is 90/60 mm Hg, pulse is 100/min, and respirations are 26/min.  Oxygen saturation is 100% on a nonrebreather mask.  The patient is awake and alert.  Diffuse blistering of the oropharyngeal mucosa is present.  Mild wheezing is noted to auscultation of the lungs.  Deep partial- and full-thickness burns are present over 45% of the body, including the face, neck, arms, and legs.  There is a circumferential deep partial-thickness burn to the left lower arm.  Radial pulses are present.  The abdomen is soft and nondistended.  Blood carboxyhemoglobin concentration is 35%.  Intravenous fluid resuscitation is initiated per standard burn protocol.
